The successful candidate will provide a high quality clinical psychology service to children, adolescent and families in child and adolescent mental health service (CAMHS), working autonomously within professional guidelines and the overall framework of the department’s policies and procedures. You will be a First Class Honours or Upper second Class degree in Psychology that provides eligibility for graduate basis for registration with British Psychological Society. The post holder will be an accredited postgraduate Doctoral level training in Clinical Psychology to meet the requirements for eligibility to register as a Chartered Clinical Psychologist with the British Psychological Society and as a member of the Health and Care Professions Council. Once registered as a member of the HCPC, the post holder should maintain registration. These qualifications confer an advanced level of theoretical and practical knowledge of a range of theoretical models and working practices.

Additional Information Specific to the Role

The post is based in a Tier 3 CAMHS Team for Children and Young People in the Motherwell/Wishaw areas. The post holder will be expected to work with the full range of presenting difficulties and to offer a specialist Clinical Psychology input to multidisciplinary casework, consultation and team supervision. The team currently works with young people from early childhood up to 16th birthday and a range of individual, family and parenting interventions is offered based on assessed individual need. There are currently 15 WTE Child Psychologists working across the CAMHS service, with 10 WTE working in the 6 locality CAMHS teams. Others are based in the Learning Disability team, the Primary Mental Health team and the CAYP (LAAC) team. All Psychologists attend a monthly Professional meeting which has a CPD component each month. A research and audit peer support group is also being developed. Professional and Line Management supervision for this post will be provided from within the MDT as there are currently 3 Clinical Psychologists based within the team

Psychologists in the service supervise trainees from both East and West of Scotland doctoral courses (offering both core and specialist placements) and the MSc course in Applied Psychology for Children and Young People. Teaching is also delivered on each of the courses. All Psychologists eligible to supervise are expected to have undertaken the NES generic supervisors training and the clinical Psychology “bolt on “when this becomes available within NHS Lanarkshire

2. JOB PURPOSE

3. ORGANISATIONAL POSITION

 To provide specialist psychological assessment and therapy to children, adolescents and families with complex psychological and mental health difficulties accessing a multidisciplinary CAMHS team in Lanarkshire. Clients may present with a wide range of disorders such as psychosis, suicidal behaviour, self injury, generalised anxiety, panic, obsessive –compulsive disorder, post traumatic stress disorder, depression, eating disorders, developmental disorders such as attention deficit hyperactivity disorder/autism, aggressive behaviour, complex bereavement issues, psychological factors relating to physical ill health and complex psychological problems arising from physical / emotional / sexual abuse and/or neglect. Many families present with complex histories where organic conditions may be interwoven with issues related to social deprivation and parental mental health difficulties.

 To work across a range of different settings including schools, out patient clinics, home visits and social work liaison settings.

 To liaise with and offer consultation to other professionals including GP’s, Paediatricians, Psychiatrists, Community Psychiatric Nurses, Social Workers, Educational Psychologists, Voluntary Sector providers and the Children’s Panel regarding the psychological well being of clients.

 To undertake research projects including the design, conduct and dissemination of results through conference presentation and journal publications.

 To provide clinical supervision to Trainee Clinical Psychologists, Associate Psychologists both qualified and in training and Assistant Psychologists in an individual and group format. To provide consultancy and supervision to Trainee Clinical and Associate Psychologists and Assistant Psychologists who are designing and conducting research projects.

 To contribute to the continuing professional development of departmental colleagues through workshops and seminars within the department.

5. MAIN DUTIES/RESPONSIBILITIES

Clinical:  To provide specialist psychological assessments of clients referred to the Tier 3 Multidisciplinary team based on the appropriate use, interpretation and incorporation of complex and conflicting data from a variety of sources including psychological and neuropsychological tests, semi structured interviews with clients, family members and other involved in the client’s care. Also using rating scales, self report measures and direct and indirect observations.  To formulate and implement specialist plans for the psychological treatment and / or management of the client’s mental health difficulties and their impact on families. This is based on an appropriate theoretical framework of the client’s problems, involves consideration of highly complex factors such as historical and developmental processes that have shaped the individual family or group and utilises techniques where there is evidence of effectiveness. Such formulations and plans are derived after consideration of a range of options and the implications for the young person and their family / carers.  To communicate with clients, families, groups and other professionals in a skilled and sensitive manner, using high levels of interpersonal skills and in an understandable format regarding information concerning the assessment, formulation and treatment plan.  To be responsible for developing and implementing a range of psychological interventions for children/adolescents, families and groups. To monitor progress and evaluate treatment options taking account of both theoretical models and therapeutic methods utilised and to reformulate where considered necessary.  To exercise autonomous professional responsibility in managing the assessment, treatment and discharge of clients.  To provide written and verbal clinical reports [and occasionally legal reports of a specialist nature] to the referring agent and relevant others on a regular basis throughout contact with the clinic. These are written in a skilled and sensitive manner and will contain information concerning the assessment, formulation and treatment plans of clients.  To exercise autonomous professional responsibility for planning, managing and prioritising clinical caseload and clinical appointments and maintaining associated case records.  To contribute directly and indirectly to a psychologically based framework of understanding and care to benefit all children/adolescents and families using the service.  To provide specialist psychological advice, guidance and consultation to other professionals contributing directly to formulation, diagnosis and treatment plan. To participate in and encourage integrated approaches to working with other relevant professionals involved in a child/young person’s care in line with current legislation for children’s services.  To assess and manage risk of suicide/harm to self and others of children / young people or other risks. To provide advice to other professionals on psychological aspects of risk assessment and risk management.  To provide a specialist clinical technical service by choosing to administer, score and interpret complex neuro psychological tests, where clinically appropriate, as an aid to diagnosis, management and treatment. A high degree of accuracy is required for both administration and interpretation as well as familiarity with the statistical foundations of assessment measures including reliability, validity, and the use of normative data. To provide neuro-psychological reports to referrers based on the data obtained.

Teaching / Training and Supervision:  To provide teaching and training on psychological aspects of care to staff working with the children / young people across a range of agencies and settings, as appropriate.  To undertake formal training in supervision and assessment of competency for doctoral Trainee Clinical Psychologist’s and trainee Clinical Associates in Applied Psychology.  To contribute specialist knowledge and skills to the pre and post qualification teaching of Clinical Psychologists, doctoral trainees, and trainee Clinical Associates in Applied Psychology as required.  To provide clinical placements for Trainee Clinical Psychologists, trainee Clinical Associates in Applied Psychology including holding clinical responsibility for the caseload of trainees and ensuring that they acquire the necessary skills, competencies and experience to provide good psychological care for these children/young people and their families.  To assess the clinical competence of doctoral Trainee Clinical Psychologists, trainee Clinical Associates in Applied Psychology and provide formal evaluation of their competency to the relevant training body (Universities of Edinburgh and Glasgow)  To provide professional and clinical supervision of Trainee Clinical Psychologists and Assistant Psychologists in both an individual and group format.  To be an active participant in team supervision within the Multidisciplinary Team, bringing an understanding of psychological theory to enhance formulation and therapeutic intervention of complex cases.  To provide consultation/supervision to staff of other professions working with the client group across a range of agencies and settings on psychological aspects of their work, as appropriate.  To contribute to the continuing professional development of other members of the team and the area wide clinical psychology service through workshops and seminars within the department.

Management / Recruitment, Policy and Service Development:  To contribute to the development, evaluation and monitoring of the operational policies and services to the Child Psychology specialty, the Multidisciplinary Team and wider CAMHS service through the use of highly developed professional skills in research, service evaluation and audit in consultation with line manager.  To advise both service and professional management on those aspects of the service where psychological and / or organisational matters need addressing.  To make proposals for and contribute to policy implementation and service development both within the Child and Adolescent service and in multi agency settings in the post holder’s designated locality.  To manage the workloads of Trainee Clinical Psychologists, Assistant Psychologists, and trainee Clinical Associates in Applied Psychology within the framework of the service’s policies and procedures.  To be involved in the short listing and interviewing of Assistant Psychologists within the CAMHS, as appropriate.

Research and Service Evaluation:  To utilise Psychological theory, evidence based literature and research/audit to support evidence based practice in work with individuals, groups and teams across the service.  To undertake appropriate research including service evaluation and complex audit within the service on an individual basis or with colleagues to help develop service provision.  To disseminate the findings of research, clinical service evaluation and audit to the wider professional by contributing to conferences and professional journals.  To provide research advice/ consultation to other professionals working with children and families, regarding audit, service evaluation and research.  To supervise research projects undertaken by Trainee Clinical Psychologists, trainee Clinical Associates in Applied Psychology and Assistant Psychologists.

Continuing Professional Development:  To receive and make appropriate use of regular clinical and professional supervision from a more senior Clinical Psychologist in accordance with good practice guidelines.  To undertake a job appraisal and review of agreed goals for continuing professional development on an annual basis.  To maintain and update knowledge and gain additional highly specialist experience and skills relevant to clinical psychology and / or in order to meet agreed goals for continuing professional development.  To develop and maintain the highest personal standards of professional practice through active participation and internal and external CPD training and development programmes.  To maintain a log of continuing professional development activities [e.g both internal and external CPD training and development programme] in accordance with professional guidelines.  To participate in and contribute to, the continuing professional development of other members of CAMHS and /or Area Wide Department of Clinical Psychology.

General:  To contribute to the development and articulation of best practice in psychology across the service. This is achieved by continuing to develop the skills of a reflexive and reflective scientist practitioner, taking part in regular professional supervision and appraisal and maintaining active engagement with current developments in the field of clinical psychology and related disciplines.  To maintain the highest standards of clinical record keeping including data entry and recording, report writing and the responsible exercise of professional self governance in accordance with professional codes of practice of the, HCPC and NHS Lanarkshire policies and procedures.  To manage personal clinical workload and appropriate professional duties and responsibilities to ensure an efficient and effective service.  To abide by the Codes of Conduct applicable to all Clinical Psychologists as required by the HCPC.  To maintain up to date knowledge of relevant legislation and national and local policies.  To contribute to the general development of the department by regularly attending and participating in department meetings.  To comply with the policies of NHS Lanarkshire and the CAMHS Service, including Health and Safety and the care and management of equipment and material.

7. SYSTEMS

 Required to maintain accurate and comprehensive clinical notes and consultation records for every client seen as defined in the operational policy, utilising both case files and electronic patient record systems.  Required to provide weekly appointment diaries for administrative purposes and complete travel claim forms  Required to provide monthly statistics on all clinical and non clinical activity for the department data base and Scottish Executive statistics.  Knowledge and competent use of computer software packages is essential such as o software packages, e.g. Microsoft Office for producing text, databases and spreadsheets o software packages e.g. SPSS for the storage, manipulation and statistical analysis of data o use of online databases and e-library resources for literature reviews. 8. DECISIONS & JUDGEMENTS

 With regards to clinical work, the post holder must decide on the most appropriate treatment option based on the complex clinical material provided by the child/young person and their family, gained during assessment. As new information is obtained, perhaps from other agencies or informants, the post holder must compare this with previously held information and assess the suitability of the therapeutic approach being undertaken. The post holder may reformulate the case depending on the new information gathered and consequently alter the treatment approach accordingly.  In relation to risk assessment the post holder is required to respond to information from the child/young person or another individual that suggests that they or another person is at risk of physical harm. This involves making appropriate enquires and informing agencies whilst also respecting confidentiality as far as possible.  Also to make decisions in relation to concerns about child protection and provide a Clinical Psychology contribution to this process.  Clinical autonomy for decisions concerning prioritisation of own caseload, treatment and discharge of cases.  Contributing towards multi agency risk assessments and risk management plans.  Contributing towards children/young people’s social work care plans, via Care Reviews and Children’s Hearing Systems.  The post-holder is expected to exercise autonomous professional responsibility on a day-to-day basis for managing personal clinical caseload including the assessment, treatment and discharge of clients.  Required to autonomously anticipate and take steps to resolve problems that occur within clinical practice.  The post-holder works autonomously within professional guidelines and the overall framework of the service’s policies and procedures including those relating to Health and Safety and Confidentiality of Information.  The assignment of clinical cases is autonomous, based on the post-holder’s own assessment of capacity and clinical needs of the locality.  The post-holder is expected to receive and make appropriate use of regular clinical supervision from a Clinical Psychology colleague in common with all Clinical Psychologists and in accordance with professional practice guidelines.  The Head of Psychology or the Team Manager may assign service evaluation audit tasks or the post-holder may work on these autonomously.  Reviews of work and professional development are undertaken by the Head of Psychology and the PMHT Team Manager through the PDP system, on a regular basis and in accordance with professional practice guidelines. 9. COMMUNICATIONS & RELATIONSHIPS

 Constantly required to communicate with children/young people and families, colleagues, and other professionals in a highly skilled and sensitive manner. Also required to consider the method of communication utilised with children and young people as this may need to be adapted depending on their developmental age and level of understanding.  Communicating with GP’s, other members of the child/young person’s network of care; paediatricians, psychiatrists, educational psychologists, teachers and social work colleagues regarding their needs is required on a regular basis to ensure efficiency of service delivery.  Communication with clients regularly involves dealing with highly complex and sensitive information. Issues of confidentiality and discretion have to be negotiated regularly and dealt with prudently and judiciously.  Conducting both planned and impromptu telephone conversations with professionals and carers regarding treatment needs  Sometimes there are barriers to communication such as sensory impairments and cognitive impairments. On occasions, an interpreter may be required to be present for those with hearing impairments or from non – English speaking communities. Questionnaires may need to be read out to those with visual impairment. Also, materials sometimes have to be adapted for patients with different cognitive abilities.  Providing written reports as required for GP’s , Social Workers, other relevant professionals, solicitors, and the Children’s Panel.  Barriers to understanding have to be negotiated and differences in opinion can occur with professionals working from different models of aetiology such as the difference between the medical model and the bio psycho social model. Differences must be explored and resolves to maintain effective communication.  Consultancy work is undertaken with colleagues outside the clinical psychology profession to inform patient management.  Teaching and training is provided to Doctoral trainee psychologists, trainee Clinical Associates in Applied Psychology, Psychology colleagues and non psychology colleagues as required.  Conference presentations and journal publications require ideas to be communicated articulately and coherently.  Regularly email departmental colleagues as well as other professionals.

Relationships:  To form and maintain appropriate therapeutic relationships with children, young people, families and carers.  The ability to foster productive working relationships with Clinical Psychology and other professional colleagues.  Work collaboratively to provide a co-ordinated and effective service in a CAMHS. 10. DEMANDS OF THE JOB (physical, mental, emotional)

 Working in different settings across a wide geographical area may require travel throughout the day therefore driving is essential.  Basic PC and keyboard skills are required on a regular basis for reports, data entry, and analysis, e mail communication and preparation of teaching material.  Significant degree of manual dexterity and integration of sensory information required for administration of neuro psychological tests such as accurate timing and co-ordination of materials whilst simultaneously attending to the child/young person’s cognitive, behavioural and emotional responses.  Clinical work requires sustained sitting in restricted situations for several hours, including sitting on low chairs at low tables with young children.  Carrying patient files, psychological test materials, play and art materials, audio visual equipment between clinical settings as required, which can be heavy. Effort required in the moving and setting up of equipment.

Mental Effort:  High degree of focused concentration and prolonged mental effort is very frequently required during patient interviews, group work and supervision.  Such concentration and mental effort can also be required in professional and clinical meetings  Non-clinical aspects of the post [teaching, training, research] require extensive analytical skills as well as high level abstract thought and conceptualisations

Emotional Demands:  Sustained attention to the emotional needs and distress of patients is emotionally very demanding. For example, patients often have sexual abuse issues, family breakdown, bereavement issues and / or anger problems and some of them regularly self harm and engage in other risk taking behaviour that the post holder has to emotionally contain and manage.  Exposed to highly emotive and distressing material on a daily basis through the provision of psychological care to children, young people and families with significant psychological and mental health difficulties.  In the clinical setting, the atmosphere can frequently be hostile with risk of verbal or physical aggression due to the nature of the problems being dealt with and the child, young person, or other family members being in distress and/or in an emotionally charged state.

Environmental Demands:  Frequently required to conduct interviews in environments such as busy outpatient settings, hospital wards or family homes which can introduce distractions and unpredictability which must be managed. 11. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB

 Clinical work involves working intensively with clients for an hour at a time, or longer if group work, listening to and/or having children/young people and their families account traumatic life experiences. These experiences can include physical, emotional, sexual abuse and /or neglect.  The post holder can also bear witness to high levels of associated emotional distress including aggression. The post holder must maintain a high degree of professionalism when faced with such emotive and distressing situations and continue to provide a stable and secure therapeutic environment.  Frequent exposure to highly distressing and emotional circumstances arising from the demanding and challenging nature of the work with this population.  Managing the anxieties, stress and sometimes hostility of carers and other colleagues involved with the patient.  Responding to urgent situations whilst holding a full caseload and managing other clinical responsibilities.  Focussed and sustained concentration required for long periods of time during clinical work with patients.  Ability to move quickly and seamlessly between different tasks such as work with patients, research activity, professional development, so as to be able to competently meet the demands of each situation.  Working within different professional networks and agencies can be time consuming and challenging at times.

12. KNOWLEDGE, TRAINING AND/OR EXPERIENCE REQUIRED TO DO THE JOB

HOURS OF WORK: 37.5 Hours

ANNUAL LEAVE: The annual leave entitlement is 27 working days, rising to 29 working days after 5 years service and 33 days after 10 years service. In addition to this you are entitled to 8 statutory holidays per annum to be taken between the period 1 April to 31 March each year. It is the practice of NHS Lanarkshire to calculate annual leave in hours.

APPLICATIONS SHOULD BE RETURNED TO [email protected] Or RECRUITMENT, HR SERVICE CENTRE, LAW HOUSE, AIRDRIE ROAD, CARLUKE ML8 5EP IMMUNISATION In order to protect patients, healthcare workers who carry out Exposure REQUIREMENTS, Prone Procedures (EPPs) as part of their role, must provide documented HEPATITIS B evidence that they do not have a blood borne virus that they may pass on to AND patients whilst carrying out EPP work (i.e. that they are not infectious for HEPATITIS C STATUS hepatitis B, hepatitis C and HIV). Blood test results must be from “identified validated samples” (IVS) and be documented as such. In some cases, it is possible for healthcare workers who have a blood borne virus, to carry out a range of EPP work. This will be assessed and monitored by an Occupational Health Consultant and the healthcare workers’ treating physician.

Healthcare workers who perform EPPs have a professional, ethical and legal duty to protect their patients and inform occupational health if they know (or suspect) that they are infected with a blood borne virus (hepatitis B, hepatitis C or HIV) now or at any time in the future. Further guidance is given by the relevant Regulatory Bodies.

DISCLOSURE OF Dependant on the nature of the post we will need to conduct a criminal CRIMINAL record check. This could be a basic or standard level or a PVG for CONVICTIONS OF regulated work. NHS STAFF In accordance with the Protection of Vulnerable Groups (Scotland) Act 2007, individuals entering regulated work for the first time will be required to join the scheme.

A Scheme Record Update will be obtained for individuals who are registered through the PVG Scheme as already doing regulated work with children or adults.

Please note: You do not have to do anything about PVG Scheme Membership at application stage. Candidates selected for appointment will be advised on how to become a PVG Scheme member prior to taking up the position.

No approach will be made without written permission of the successful applicant, who will be asked to sign and complete the relevant Disclosure Scotland PVG Scheme application, giving authorisation for the check to be undertaken.

The cost of the PVG Scheme membership to the individual is a one off payment of £59. NHS Lanarkshire reserves the right, following consultation with you, to recover from you any payment made in respect of PVG Membership
