Free For 2 (FF2) Application Form EC - Guidance Notes (April 2017) Section 1 – Declaration of facts Line No. Notes Please complete with the name of the setting, childminder or children’s centre and the telephone number where the application was completed. 1 - 2 This information will allow us to identify you should we need to get in touch. Please write clearly and use a suitable email address that you check regularly. Confirmation of eligibility will be sent to this address and incorrect information will delay you finding out if the 3 child is eligible. This is NOT the parent’s email address. 4 - 5 This information must be provided as we are unable to make a payment without it. 6 Legal first name followed by surname (as shown on birth certificate or passport) 7 Please specify if the child is male or female. Birth certificate or passport can be used to evidence that the DOB is correct. Please make sure that the DOB falls within the correct DOB range for the current term. Call the FF2 team if you are unsure of the current DOB range. Please note: we cannot process an application form until the child is in the correct DOB range, so do not send it to us early. 8 Please note: we can no longer accept the red book as proof of DOB 9 This should be the name of the person who evidenced the child’s date of birth. 10 If the parent knows which provider they wish to use, please insert details here.

Section 2 – Benefits / Credits - to be completed in respect of the person named in Section 3 You may wish to ask if the household is claiming a free school meal for an older sibling as this is 11 an indicator that the child named in Section 1 could be eligible for a free place. For tax credits, please ensure that the claimant is aware that the household annual income must 12 be no more than £16,190. The full Universal Credit service for all claimants is gradually being rolled out in Kent and should 13 be complete by September 2018. If a parent is accessing Care to Learn, the school/college/university must be informed that the child is accessing a funded 2 year old place so that additional hours above their free entitlement Info can be funded. Some families who meet the 2015 criteria may already be paying for childcare and claiming tax credits as a result. Families who are in receipt of Child Tax Credits, who then become eligible for a free place, must inform HMRC within one month if:  they normally pay fixed weekly childcare costs and the actual costs go down by £10 a week or more, and the change applies in each week for 4 weeks in a row  they pay the same amount every month or different amounts for childcare at different times, for example, they pay more in the school holidays than they do during term time and their new average weekly costs go down by £10 or more  they stop paying childcare costs. It is important that families advise HMRC / Tax Credits of any changes in their circumstances within one month. If they do not, they may build up an overpayment which they will have to pay Info back. They might also have to pay a penalty of up to £300.

Section 3 – Personal details (parent / carer information) Ensure the claimant is aware that this must be the name that is registered with HMRC and/or 14 DWP to receive benefit or credit. Please complete with the FULL postal address and ensure that this is the address known to 15 HMRC. Incorrect information will delay the eligibility checking process. 16 In the format : DD / MM / YYYY. National Insurance Number: In the format : 2 Letters / 6 Numbers / 1 Letter This MUST be the claimant’s National Insurance Number (NINO). National Asylum Seeker Service Reference: If they do not have a NINO, they may have a National Asylum Seeker Service reference (NASS). This number can be found on the right hand side of the asylum seeker letter of support from the Home Office. It has 9 digits and is usually 17 shown in the format : YY / MM / 12345. 18 - 19 Always useful in case we need to clarify details with the claimant. 20 Please ask the parent to sign and date the form before you return it to us. Please encourage the parent / carer to give their permission for this information to be used to Info check entitlement to EYPP.
