2. When was this RFQ placed on the Chest and when was the closing date for respon ses? RESPONSE The RFQ was published on the e-tendering portal The Chest 17:35 11/7/14 and the closing date was 09:30 23/7/14

3. Please supply the names of all 16 Organisations that the RFQ's were sent to. Please supply the total agreed Price Increase that this Contractor received as a result of the overcharging incident RESPONSE Variations to costs charged by the contractor may reflect changes in volume of pupil numbers or routes on a daily basis. Therefore it is not possible to supply this information.

5. a) How did the overcharging incident come to light? RESPONSE The Council’s internal contract monitoring arrangements identified this matter when the first invoice was received.

b) Was a complaint received by the Authority? RESPONSE A whistleblowing allegation has been received by the Council on this issue and is currently being investigated.

6. Please supply any Internal Audit Reports that formed part of this investigation or pr ocess. RESPONSE A whistleblowing allegation has been received by the Council on this issue and is currently being investigated.

7. In Section 4.3 please provide the names of the Council Officers and the Council’s Procurement Manager who evaluated the responses. RESPONSE The responses were evaluated by the Council’s Inclusion Service Manager and the Lead Education Officer SEN-Inclusion. The Council’s Procurement Manager does not evaluate responses.

8. Please provide details of all previous RFQ's that have been placed for Home To Sc hool, Ad Hoc Taxi or Adult Social Care for the last 5 years. Please provide: a. title/ name / number of RFQ, b. the value of each RFQ c. the names of the organisations that were invited for each RFQ d.the winning Respondent for each RFQ e. the method by which each RFQ was advertised
