Background
Healthy Families America requires that each site prepare an annual Home Visit Completion Analysis. Healthy Families Florida requires that this annual analysis be submitted to Central Office as a contract deliverable due in mid-November. The analysis is included in the Biannual Service Review every odd year, and is a stand-alone report on even years.
Excerpted from HFF Biannual Service Review Guide 1. Title Page for Home Visit Completion A. Narrative explaining the home visit completion rate for the project for the reporting period. This information can be found in the Outcomes/Process Measures/Outcomes and Process Measures (four quarters) or (quarterly). The information should be provided by team and by FSW. Because the Performance Management Reports do not provide historical FSW assignment data (home visits done by previous FSWs now show up as having been done by the currently assigned FSW), information from supervision documentation regarding home visit completion may also be included.
The narrative should include discussions regarding challenges whether by project, team or FSW. Discuss any barriers to home visit completion as well as any strategies used to increase home visit completion during the reporting period. Review the strategies/plan from the previous Annual Service Review and report on the implementation and effectiveness of those strategies.
B. Provide the plan for increasing home visit completion. This should address challenges mentioned above. A plan is not required if each home visitor’s home visit completion is 90 percent or higher.

Helpful Tips • HV Completion Rates for FY 15-16 should be the basis of the analysis. • If HV Completion was included on your site’s QIP at any time during FY 15-16, include an explanation of why, what improvement action steps were identified, completed and results.
• Compare actions steps from Home Visit Completion Analysis section for the BSR that was submitted in Nov 2015. Address what was in the plan, what action steps were completed, what actions steps were not completed and why, and results.
• Reports should be run in the HFF Performance Management System for Outcomes And Process Measures (4 quarters), which can be sorted by “All Satellites” (entire site), “Satellite”, supervisor and individual staff member.
