Part 2: Activity #2 Texas Department of State Health Services Family Centered Case Management -- Community Partnerships “Connecting Children and Families to Supports and Services”
The purpose of this activity is to introduce you to the Texas Department of State Health Services (DSHS) which offers services that may benefit ECI children and families.
This section, “Connecting Children and Families to Supports and Services,” will provide you with a framework to conceptualize the extensive system of supports and services that link families to needed services.
Keep in mind that the agencies you are introduced to reflect SOME but not all resources. Learning about all of the available resources for families will be an ongoing process.
Directions: Explore the Texas Department of State Health Services websites provided in this activity. When you have finished, complete the matching exercise below and give to your supervisor.
You have already been introduced to the Case Management and Pregnant Women’s program the Texas Early Hearing Detection and Intervention program, and Texas Health Steps in the DSHS portion of the module. Because both of these agencies provide services that directly impact your work with families, you will complete more work on these programs in this activity.
Many of the websites you will visit are related to finding health care for low income families. It is important that you have an awareness of what is available for families in your area. As a service coordinator, you will want to support families who have a need in this area.
As you explore the websites, gather information to learn the following: 1. service(s) and/or supports that each agency provides to families and children, 2. relevant contact information, 3. procedures or processes to successfully link families with identified services, and 4. how each agency supports your work as a Service Coordinator.
