TB RELEASE PLANNING CHECKLIST Instructions: Use this checklist when beginning the release planning process for a high-priority inmate. (Note: begin planning for release at first notification that this is a high-priority inmate). Check each task once COMPLETED in the column to the left of each step. Indicate any follow-up needs, initial, and date.

COMPLETED STEPS/PROCEDURES OF RELEASE PLANNING PROCESS INITIALS DATE FOLLOW-UP (IF ANY) (CHECK IF YES) Identify members of the Release Planning Team. Participants of the group should include the following: a. Custody Administration/designee b. Classifications Supervisor/designee c. Medical Administration/designee (often takes the lead in this process) d. Public Health TB Program Manager/designee Convene the team to discuss the current release process and develop a revised plan as needed for the safe release of high priority inmates. Medical should place a notification (stop sign, or other means of identification/ communication) in the inmate's release/classification folder and medical record to ensure inmate returns to medical prior to release or transfer. Identify inmate’s forwarding home address and healthcare provider (i.e., clinic or primary care physician). NOTE:: Complete the tool TB Release Planning form. Identify the inmates estimated release/court date from Classifications or inmate records department/custody. Notify necessary Health Department staff of upcoming potential release date. Do NOT discuss the inmate’s release or court date with others not involved in this process, especially the inmate.

TIP: inmates may be released or transferred directly from the courthouse pending trial and may not return to the facility. If the inmate is awaiting trial, note the following on the TB Release Planning form:  Date of next court visit  All charges that inmate will be arraigned on  If ongoing trial, other pertinent information (e.g., public defender/lawyer's name, name of judge, name of state attorney prosecuting case, etc.). Once custody identifies the inmate’s release, inmate is seen by medical for final evaluation and instructions for follow-up care. Provide inmate with health department contact information and medication, as needed. Instruct inmate to follow-up within 3 days. Once the inmate is released, or no longer requires follow-up care, remove the notification (Stop Sign or other document) from the inmate's Release/ Classification Folder.

Evaluate the Release Planning process with the Team and revise plan as needed.

Below are additional tips to facilitate the release of high-priority inmates into the community or transfer to other facility.  Identify or develop a system of notification that follows the inmate from intake to release (i.e., a computer generated link/form or contact card, or stop sign) to advise staff that this inmate will require medical follow-up prior to release or transfer.  The team should formalize a policy and procedure once this process is implemented and refined. Educate personnel on the process.  Assure isolation for inmate if active case of TB.  Begin release planning immediately and document steps taken on TB Release Planning Summary Form.
