JOB DESCRIPTION

Job Title: Health Care Assistant

Grade: 3

Responsible to: Partners for clinical care and management. Practice Manager for personnel and administration

Summary of duties:

Assist in the provision of nursing services to patients attending the Practice. This will include undertaking diagnostic tests, preventative health care, screening and patient education.

Main Duties and Responsibilities:

1 Assessment, screening and treatment services and health education advice.

a) Provide general and specific health screenings to the practice patients within agreed protocols with referral to a nurse or a GP as necessary.

b) Assist the GP in the provision of private medicals (HGV, local businesses etc.)

c) Provide follow-up advice for smoking cessation and weight management.

d) Perform registration checks for new patients

e) Assist in minor operations undertaken at the Practice.

2 Pathological specimens and investigatory procedures

a) Undertake the collection of pathological specimens including blood samples and swabs.

b) Undertake routine diagnostic tests such as urinalysis, ECG recording, spirometry and peak flows.

3 Administrative and Professional Responsibilities

a) Participate in the administrative and professional responsibilities of the practice team

b) Ensure accurate notes of all consultations and treatments are recorded in the patient’s computerised records.

1 c) Participation in clinical audit, practice research projects and significant event analyses.

d) Implement infection control requirements.

e) Assist in Health Promotion displays.

f) Assist in ensuring that clinical rooms are stocked, as required.

g) Carry out equipment check according to the Practice's policy or with the agreement of a practice nurse.

4 Liaison

Maintain effective liaison with all other members of the health care team, with appropriate regard to confidentiality.

5 Professional Development

a) Participate in the development and implementation of a personal and professional development plan.

b) Maintain continued education by attendance at course and study days.

6 Other

a) Follow Practice policies and procedures at all times.

b) Ensure security of data and information in accordance with the Data Protection Act and the Practice's policies and procedures.

c) Promote and maintain own and other's health, safety and security, in accordance with legislation and the Practice's Health and Safety and Security policies.

d) Support the equality, diversity and rights of patients, carers and colleagues, to include:  acting in a manner that recognises the importance of people's rights, interpreting them in a way that is consistent with the Practice's procedures and policies and current legislation,  respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues  behaving in a manner which is welcoming to, and of, the individual; is non- judgmental and respects their circumstances, feelings, priorities and rights.

e) Participate in the Practice's staff appraisal system.

f) Undertake any other duties that are considered to be appropriate to the post.
