
<p> Sandy Lane Surgery Sandy Lane Leyland PR25 2EB</p><p>JOB DESCRIPTION</p><p>Job Title: Health Care Assistant</p><p>Grade: 3</p><p>Responsible to: Partners for clinical care and management. Practice Manager for personnel and administration</p><p>Summary of duties: </p><p>Assist in the provision of nursing services to patients attending the Practice. This will include undertaking diagnostic tests, preventative health care, screening and patient education. </p><p>Main Duties and Responsibilities:</p><p>1 Assessment, screening and treatment services and health education advice.</p><p> a) Provide general and specific health screenings to the practice patients within agreed protocols with referral to a nurse or a GP as necessary.</p><p> b) Assist the GP in the provision of private medicals (HGV, local businesses etc.)</p><p> c) Provide follow-up advice for smoking cessation and weight management.</p><p> d) Perform registration checks for new patients</p><p> e) Assist in minor operations undertaken at the Practice.</p><p>2 Pathological specimens and investigatory procedures</p><p> a) Undertake the collection of pathological specimens including blood samples and swabs.</p><p> b) Undertake routine diagnostic tests such as urinalysis, ECG recording, spirometry and peak flows.</p><p>3 Administrative and Professional Responsibilities</p><p> a) Participate in the administrative and professional responsibilities of the practice team</p><p> b) Ensure accurate notes of all consultations and treatments are recorded in the patient’s computerised records.</p><p>1 c) Participation in clinical audit, practice research projects and significant event analyses.</p><p> d) Implement infection control requirements.</p><p> e) Assist in Health Promotion displays.</p><p> f) Assist in ensuring that clinical rooms are stocked, as required.</p><p> g) Carry out equipment check according to the Practice's policy or with the agreement of a practice nurse.</p><p>4 Liaison</p><p>Maintain effective liaison with all other members of the health care team, with appropriate regard to confidentiality.</p><p>5 Professional Development</p><p> a) Participate in the development and implementation of a personal and professional development plan.</p><p> b) Maintain continued education by attendance at course and study days.</p><p>6 Other</p><p> a) Follow Practice policies and procedures at all times.</p><p> b) Ensure security of data and information in accordance with the Data Protection Act and the Practice's policies and procedures.</p><p> c) Promote and maintain own and other's health, safety and security, in accordance with legislation and the Practice's Health and Safety and Security policies.</p><p> d) Support the equality, diversity and rights of patients, carers and colleagues, to include: acting in a manner that recognises the importance of people's rights, interpreting them in a way that is consistent with the Practice's procedures and policies and current legislation, respecting the privacy, dignity, needs and beliefs of patients, carers and colleagues behaving in a manner which is welcoming to, and of, the individual; is non- judgmental and respects their circumstances, feelings, priorities and rights.</p><p> e) Participate in the Practice's staff appraisal system.</p><p> f) Undertake any other duties that are considered to be appropriate to the post.</p><p>2 SANDY LANE SURGERY HEALTH CARE ASSISTANT PERSON SPECIFICATION</p><p>A Education and Qualifications Essential Desirable Means of Assessment Good general education NVQ2 Health & Social Care CV NVQ3 Health and Social Care</p><p>B Knowledge and Experience Essential Desirable Means of Assessment Basic computer Working with EMIS, the CV knowledge practice's clinical computer system Using Microsoft Outlook and Word</p><p>C Skills and Abilities Essential Desirable Means of Assessment Good communication Self-motivating Interview skills Exercise good judgement Good team working Good interpersonal skills Work accurately Be flexible Work within guidelines</p><p>D Special Requirements Essential Desirable Means of Assessment Possess high work Interview standards Able to pass pre- Occupational employment medical Health Assessment Able to obtain satisfactory CRB clearance</p><p>3</p>
