POST TITLE: DENTAL FOUNDATION TRAINING POST DF2

RESPONSIBLE TO: Head of Service, Berkshire Community Dental Service.

HOURS: 37.5 hours per week.

10. Berkshire Community Dental Service provides high quality dental care for a wide spectrum of people within Berkshire who are unable to access care in general dental practice. The care is provided in a variety of settings including dental clinics, mobile dental units, schools and day centres, Dental Access Centres, prisons, remand centres and other secure establishments and hospitals. Domiciliary care is also provided in care, residential and nursing homes and private residences. The service also provides Oral Health Promotion and epidemiology.

JOB SUMMARY

The dentist will fulfill duties within the Community Dental Service. The dentist will discharge responsibilities and undertake duties necessary for the proper fulfillment of service contract(s) into which the Trust has entered. These will cover the main areas set out below, but may be modified as necessitated by the service agreements or service contracts then in operation.

To provide high quality primary clinical dental care for child and adult patients with special needs who fulfill the criteria for referral and treatment within the service and who cannot be accommodated by a General Dental Practitioner.

The care includes the full range of restorative and preventive NHS treatments from clinics, dental mobiles, other health care establishments, schools and day centres, prisons, remand centres and other secure establishments and on a domiciliary basis where appropriate.

This may involve treatment under sedation or general anaesthetic utilising the dentist’s personal skills and training or working with appropriately trained others to provide such care. The dentist may be required to participate in health promotion, screening, dental epidemiological surveys and supervision of professionals complimentary to dentistry.

To take part in the Berkshire Dental Access Centre scheme and provide primary dental care for patients not registered or unable to receive emergency care with a General Dental Practitioner under NHS arrangements. This service includes the following elements: (1) Examination and first line treatment to palliate and control any immediate signs and symptoms; (2) Assessment of each patient as to whether further treatment is indicated, and if so in which healthcare settings, according to agreed criteria, it should be carried out. This may include the Dental Access Centre, the General Dental Service the Community Dental Service, General Medical Practice or hospital.

The dentist, who will be registered with the General Dental Council, will be expected to work with all patients who receive care in the service and to perform all duties expected of dentists within the clinical governance agenda.

The post holder will need Criminal Records Bureau clearance and will have to undertake further security clearance before working in a high security facility.

The dentist will actively participate in clinical governance, audit, training programmes and professional development to improve and maintain the quality of service provision.

Normal working hours are Monday to Friday, 08.30 – 12.30 and 13.30 – 17.00.

These duties and hours of working are subject to change depending on the needs of the service and the post holder will be expected to be flexible about the places and times of working, within the agreed overall number of hours, to ensure that a clinical service is maintained at designated periods both within and outside normal working hours on both weekdays and at weekends.

TRAINING

The trainee will have the opportunity to treat patients a wide variety of treatment needs in a range of settings. They will have the opportunity to assist at general anaesthetic session for both paediatric extractions and full care for patients with special needs. They will also have the opportunity to observe or assist at sessions for patients requiring treatment under sedation and in secure establishments.

Days will be allocated for training/study purposes and clinical and educational support provided. And the trainee will be expected to attend tutorials and other specified educational sessions and use allocated study time effectively.Additional training will be provided where necessary to enable the dentist to meet the needs of the service. The trainee will be expected to undertake necessary training and development to comply with national, professional and Trust requirements and take part in all aspects of clinical governance including GDC lifelong learning, clinical audit and clinical supervision.

The trainee will complete a learning agreement with their training supervisor in the first few weeks of the appointment. Regular approval will be undertaken thereafter. The electronic portfolio is supported and trainees are expected to complete this on a regular basis as part of their on-going learning.

ACCOUNTABILITY

2 Formal accountability is to the Head of Service of the Berkshire Community Dental Service. On a day to day basis, the dentist carries responsibility for all other staff working in the same clinic, under the overall supervision of the appropriate Senior Dental Officer. The dentist is professionally responsible for his/her clinical work.

WORKING RELATIONSHIPS

LIAISON

 Members of the community service, hospital dental staff, general dental practitioners, general medical practitioners and other NHS personnel, and local authority staff (including education and social services)  Patients, their carers and relatives, members of the public and of welfare and other relevant organisations.

MAIN DUTIES AND RESPONSIBILITIES

Clinical/ Professional  Comprehensive oral health care of child and adult patients who fulfill the criteria for acceptance into the service  Advising on oral health issues, and providing preventive care as required  The appropriate recall, transfer or discharge of patients at the end of their agreed course of treatment  Supervision of the day-to-day organisation and management of the dental premises at which the dentist works, with particular reference to:  The management of the clinics to ensure that all clinical commitments are scheduled and completed on time  The maintenance of proper standards appropriate to a clinical area, including control of infection procedures  The efficient and effective use of resources  The maintenance of confidentiality of personal health data, including patient records, in accordance with established procedures  The maintenance of high standards of conduct in relation to patients and the general public  The proper application of operational procedures issued regarding the provision of dental services.  Supervision of the dental support staff working there  Referral of suitable patients to a dental therapist when appropriate and available  Supervision and monitoring of dental therapists in accordance with the ancillary dental workers regulations and any relevant local procedures  Ensuring that clinical information is collected in the prescribed paper or electronic format for prompt transmission to the business services authority or other appropriate body  Ensuring that appropriate payment is collected on behalf of patients for treatment received  Ensuring financial probity of such payments  Ensuring adherence to statutory regulations, regional and local health authority and trust requirements, including departmental protocols and procedures  Participation in the clinical governance scheme of the employing trust, including continuing professional education, quality assurance, risk management, clinical audit, clinical supervision and performance appraisal  Participation in the development and training of other grades of staff, as required  Providing information and completing returns promptly as required by the service and the trust  Ensuring adherence to statutory regulations and trust requirements, including departmental protocols and procedures  Other duties appropriate to the grade, and including special projects, as determined by the head of service  As a member of staff within the service, the postholder will be required to accept their full share of responsibility for the performance of the service

13. ADDITIONAL REQUIREMENTS

 To take appropriate responsibility, where necessary, to ensure the Trust’s corporate strategies and objectives are achieved and contribute to the smooth running of the dental service.  To have access to personal transport to visit all areas within the Trust. Payment for the use of the car will be made in accordance with the appropriate terms and conditions of service.  To provide and maintain excellent customer care standards, assessing and understanding the needs of patients to assist with form completion, booking an interpreter, liaising with key workers and other health professionals.  To undertake other duties to meet the changing needs and priorities of the Community Dental Service, as determined by your manager and in accordance with the grade of the post.  To maintain and extend a good working up-to-date knowledge of dental procedures and to identify areas of personal development to ensure high standards are achieved. PROFESSIONAL INDEMNITY

Each dentist is covered by the employing NHS Trust with regard to claims for negligence. However, it is recommended that he or she should maintain membership of a defence organization to a level personally appropriate so that independent and confidential advice is available on all professional matters.

4 OTHER TERMS AND CONDITIONS

Other terms and conditions are currently those laid down in the Salaried Dentists in NHS Primary Care contract, although the Trust reserves the right to amend them.

14. G ENERAL

 This job description is not exhaustive and can be altered in consultation with the post holder.

 Any other duties which may be requested by the line manager in order to facilitate the smooth running of the hospital.

Reports to: DF2 Trainer/Clinical Director

Responsible to: Clinical Director for overall professional standards and to Educational Supervisor for routine training and clinical matters.

Post Role: To provide comprehensive dental care to priority client groups.

Duties and Responsibilities

1* The provision of comprehensive dental care to adults and children with learning disabilities, medical or physical disabilities and to the elderly or housebound or special care.

2* The provision of comprehensive dental care to those school and pre school children whose parents/guardians consent to treatment being provided who would not otherwise receive care from the General Dental Services, due to severe anxiety, behavioural management problems, as gateway for treatment under GA.

3* The Dental Officer, where appropriate, will allocate work to Dental Therapists or Hygienists within their recognised field of capability and with due regard to the regulations applicable to these Dental Care Professionals.

4* The Dental Officer may have the opportunity to be involved in screening and epidemiological surveys.

5* The Dental Officer will have the opportunity to deliver care in a variety of different settings. This may vary from domiciliary care in residential homes, to Mobile Surgeries, Immigration Detention Centre as well as Health centres and Hospitals.

6* The Dental officer may have the opportunity to Contribute and participate, in external Oral Health Promotion programmes.

7* There will be regular sessions for the DF2 post holder to carry work out under General Anaesthetic, IV sedation and Inhalational sedation. GA/IV Sedation on a Wednesday morning shared clinic at Poswillo Unit.

8* There will be opportunities to work with clinicians with interests in Prosthetics and Endodontics.

9* The DF2 post holder will have the opportunity to work in the Access centre environment providing unscheduled care to a variety of patient groups.

8 In relation to the clinical care of individual patients the Dental Officer enjoys full clinical freedom.

12 22. The Department

The department at Wexham Park hospital provides a comprehensive Oral & Maxillofacial surgery (OMFS) diagnostic and treatment service to a local population of approximately 350,000 and is linked with the Regional OMFS unit at Northwick Park hospital, Harrow in a 'hub and spoke' fashion. Out-patient clinics are held at Wexham Park hospital, Slough, Heatherwood hospital, Ascot, King Edward hospital, Windsor and St Marks's hospital, Maidenhead. Day surgery under general anaesthetic and IV sedation is carried out at Wexham Park and Heatherwood hospitals. Minor oral surgery under local anaesthetic is carried out mainly at Wexham Park hospital and also at Heatherwood hospital. All elective In-patient and emergency admissions are accommodated at Northwick Park hospital.

23. Infection Control

Infection prevention is the responsibility of each individual. Employees are required to strictly adhere to the infection control policy Tpp124 and it appendices, and should actively encourage other staff and visitors to comply.

Main Duties and Responsibilities and Daily Commitments

24. Clinical

The post holder will be expected to maintain a high quality level of service for elective and emergency patients. He/she will participate with colleagues in:

1. Assessment and management of maxillofacial trauma and emergency patients at Wexham Park hospital and co-ordination of transfer of patients who require admission to Northwick Park hospital.

2. Outpatient Clinics at Wexham Park Hospital - General clinics - Orthognathic clinics - Minor oral surgery under local anaesthetic - Pre-assessment clinics

3. Day surgery lists at Wexham Park Hospital

4. Ward duties, Management of emergency patients and assistance in theatre at Northwick Park hospital - Head & Neck oncology - Orthognathic surgery - Salivary gland surgery - Facial trauma - Dentoalveolar

5. Research & Audit - Publications for professional journals - Case presentations for local and regional postgraduate meetings - Commitment to audit and clinical governance The post holder will alternate on a weekly basis between Wexham Park Hospital and Northwick Park Hospital, Harrow which is the regional OMFS centre. Duties at Northwick Park Hospital include ward-rounds, looking after ward patients including clerking, cannulation, taking bloods etc, assisting in theatre with major cases and attending A&E for maxillofacial and dental trauma and emergencies. When at Wexham Park Hospital, the post holder will be involved in performing minor oral surgery under supervision, assisting in theatre (Day surgery) and attending out-patient clinics. There is no on-call commitment with this post.

Administration

The post holder will be expected to maintain high standards of communication and documentation in relation to patient care, including maintaining medical records, the preparation of discharge summaries and clinic letters and maintenance and collection of statistical data. Secretarial support is provided

STUDY AND TRAINING

25. Training

The post holder will contribute to the regular teaching and journal meetings in the department and will be encouraged to undertake a small audit project during the six months; this should only take a few hours of your time. There are opportunities to undertake research if required.

Attendance at the monthly academic half-day at WPH and the regional SHO study days in Oxford is mandatory. There is a monthly journal club/clinico-pathological conference/radiology meeting held at Northwick Park hospital where interesting cases are discussed. Formal teaching sessions are provided on a weekly basis.

26.

Library Facilities

The Postgraduate Medical Centre at Wexham Park hospital contains an excellent Library with a full time Librarian who is always willing to help and advise Medical staff. A small collection of books and journals is available in the Accident and Emergency Department.

Study leave may be granted following approval by the District Clinical Tutor but only for recognised courses/examinations, and at the discretion of the Consultants.

Postgraduate Medical Education/Courses available:

Wexham Park Hospital runs its own ATLS, ALS and APLS courses. The Postgraduate Medical Centre is extremely active and runs a large number of courses, lectures and tutorials, many of which may be of interest to you. All are available to doctors working in the East Berkshire District.

Specialty Services Provided in the Unit/Trust

Medical examination is not required prior to appointment, but it will be necessary for you to complete a questionnaire for Occupational Health clearance. In view of the unpredictable nature of accident and emergency work, DF2’s/SHO's are recommended to have had a recent chest x-ray in addition to being fully immunised against Tetanus and Hepatitis B.

Requests for annual leave must be tendered prior to commencement of the post or during the first fortnight so that the rota for the whole six-month period can be published as soon as possible. It must always be accepted that in occasional emergencies and unforeseen circumstances, a DF2/ SHO may be asked to undertake additional tasks. In these instances, claims for extra payment will be met without question.

1. MAIN PURPOSE OF JOB

Provision of an efficient and smooth running dental service to patients requiring special care dentistry

2. GENERAL DUTIES

 Undertake a full range of duties to meet the needs of the service and to address the UK Dental Foundation Programme curriculum requirements as identified in an agreed personal development plan.

 Provide a full range of dental care to patients who have experienced difficulty in accessing care in the general dental service because of their special needs e.g. physical disability, learning difficulty. This may include patients in mental health units.

 Provide dental care to patients appropriately referred to the service by other health care professionals. Including nervous and phobic adults and children.

 Provide domiciliary dental care, where appropriate.

 Provide treatment under inhalational sedation, intravenous sedation and general anaesthesia. Appropriate training and support given depending on previous experience.

 Opportunities within the service to work with clinicians on various specialists lists such as Special care, Paediatric Dentistry and Oral surgery.

4. PROFESSIONAL

 Provide dental care in line with local and national policies and procedures. This includes keeping of accurate and contemporaneous notes, use of dental software, adherence to NICE guidelines etc.

 Liaise with colleagues in hospitals, primary care and local authorities to ensure and improve the quality of patient care.

 Undertake necessary training and development to comply with national, professional and Trust requirements.

 Take part in all aspects of clinical governance including GDC lifelong learning, clinical audit and clinical supervision.

 Maintain close working relationships with other disciplines as related to these duties.

 Take part in epidemiological studies, dental screening and health promotion activities as required to meet the dental public health requirements of the Primary Care Trust.

 Provide statistical and other information to the Business Services Authority and PCT directors as required.

 Be responsible for data collection and reports as required for the Salaried Dental Service.

 Prepare and attend tutorials and other specified educational sessions and use allocated study time effectively.

 Sit a minimum of one part of the MJDF examination during the tenure of the post.

 Other such duties as may be delegated.

5. MANAGERIAL

 Manage the day to day activities of the dental surgery in conjunction with the other members of the team.  Supervise and delegate appropriate tasks to the ancillary staff in accordance with current General Dental Council regulations where applicable.

6. PERSONAL RESPONSIBILITY

In order to fulfil the job responsibilities and the environment in which these responsibilities are carried out the job holder is required to:

 Keep accurate and professional records and information about staff, patients and clinical care, using the document based and computer based systems in operation in the Trust.

 Take responsibility in the team for the security and confidentiality of personal and sensitive information about the PCT and its staff, partners and patients, through adherence to Information Policy and authorized use of smartcards and passwords.

 Hold a current driving licence and to provide a car for use as required in the performance of their duties. Payment for the use of the car will be made in accordance with the appropriate terms and conditions of service.

 The post holder will be covered by the employing Authority in relation to claims for negligence but it is advisable for individuals to maintain membership of a Defence Organisation so that independent advice is available to them on all matters relating to their professional activity.

7. ROLE DIMENSIONS

 The post holder will be expected to attend dental staff meetings and audit meetings.

 The post holder may be expected to attend other meetings as deemed relevant to the position or as delegated by the Assistant or Clinical Director

 Staff are required to work within a number of sites. As some of these sites are not easily served by public transport, the post-holder will be expected to make necessary travel arrangements to meet this need.

8. NOTES

. The post holder will maintain confidentiality where appropriate in relation to PCT matters keeping any information about individual patients confidential, subject to the Public Interest Disclosure Act 1998. . JOB SUMMARY This is a clinical training post.

The post will predominantly be based at the East Oxford Clinic but is likely to LOCATION involve outreach to additional primary or secondary care based localities. PRINCIPAL RESPONSIBILITIES

Work of the Department The department of maxillofacial surgery provides a service to the Portsmouth, South East Hampshire Health Authority and the Isle of Wight, which has a population of around 700,000. All aspects of the speciality are covered and there is close liaison with the department of Orthodontics with whom joint clinics are held weekly. Other joint clinics are held with Oncology and ENT, the Pain Clinic, Plastic Surgery (Cleft Clinic), Restorative Dentistry and there is a good liaison with other hospital departments. The Maxillofacial OPD Department is based at Queen Alexandra Hospital where the in-patients are also managed. QAH also houses the Accident & Emergency Department. The Department has a commitment to the care of the handicapped, and severely physically and mentally handicapped patients are assessed on weekly special assessment clinics by a Consultant Anaesthetist and the Associate Specialist. OMFS have their own in-patient theatre in addition to daycare surgery. A significant Trauma, Oncology and Orthognathic surgery workload is undertaken.

Duties of the Post The Senior House Officer (Foundation Trainee) is expected to participate in all aspects of the work of the department under the general direction of the Consultants, including out- patient Clinics, operating sessions and the management of patients presenting to the A&E Unit with dental / maxillofacial trauma and emergencies as well as the care of in patients varying from dentoalveolar surgery through to complex reconstructions of the face, head and neck. The trainee will be well supported by senior cover in the form of Specialist Registrars and Consultants. The Senior House Officer will participate in a partial shift on call rota which is currently compliant with the European Working Time Directive. All junior staff will be expected medical audit.

Hours Hours and banding are currently in line with the European Working Time Directive with fixed annual leave. The department provides 24/7 cover with the trainee being on site when on call.

Teaching and Study Teaching is primarily clinical but seminars are organised within the department and Teaching is protected and attendance is expected. There are postgraduate programmes in all aspects of medicine and surgery at the Postgraduate Medical Centre and informal attachments may be arranged with other firms for preparation for the final MFDS examination. Excellent medical library facilities are available at Queen Alexandra Hospital. The post is recognised by the Royal College of Surgeons of England under the MFDS and MRCS regulations, and the Royal College of Surgeons of Edinburgh under the MRCS (maxillofacial) regulations. A monthly Clinical Governance Meeting takes place in the department which encompasses audit presentations, and journal club. Professor Brennan has an active research programme ranging from clinical audit and prospective randomised studies to laboratory based research with protected time. There are excellent opportunities for research and many of our SHO/DF2s have accepted peer reviewed publications as well as the opportunity to present regionally and nationally during their appointment. The SHOs/DF2s will have opportunity for local and regional induction incorporating the running of the department as well as topics covering care of the acutely ill patient. A trauma plating course is arranged every six months.

Learning Agreements All staff will complete a learning agreement with their training supervisor in the first few weeks of the appointment. Regular approval will be undertaken thereafter. The electronic

26 portfolio is supported and trainees are expected to complete this on a regular basis as part of their ongoing learning.

Introduction

University Hospital Southampton NHS Foundation Trust provides services to some 1.3 million people living in Southampton and south Hampshire, plus specialist services such as neurosciences to more than 3 million people in central southern England. It is also a major centre for teaching and research in association with the University of Southampton and partners including the Medical Research Council and Wellcome Trust. UHSFT gained foundation trust status on 1 October 2011. (www.uhs.nhs.uk).

Southampton General Hospital is the Trust’s largest location, with a great number of specialist services based here, including Oral and Maxillofacial Surgery, and Orthodontics. Emergency and critical care is provided in the hospital’s special intensive care units, operating theatres, acute medicine unit and emergency (A&E) department. The hospital is the south coast's only paediatric and adult Major Trauma Centre, with an on site helipad. It also hosts out-patient clinics, diagnostic and treatment work, surgery, research, education and training.

The Royal South Hants Hospital is near the centre of Southampton. Located here is the Southampton NHS Treatment Centre which provides specialist treatment in 9 specialties, including Oral Surgery. The Treatment Centre provides a wide range of day and inpatient elective surgeries, and diagnostic procedures not requiring critical care. This Independent Sector Treatment Centre (ISTC) is run on behalf of the NHS by an independent company, Care UK, but treats solely NHS patients. (www.southamptontreatmentcentre.nhs.uk).

We cover a large area of Hampshire which stretches from the south coast up to Salisbury in the west, and Andover in the north. A significant number of patients are referred from the Channel Isles for major Maxillofacial surgery.

DF2/SHO Duties

There are 5 DF2/SHOs and 1 surgical FY2 doctor. The job plan consists of a six week rotation, with different duties for each of these six weeks. On-call cover is provided at SGH during the day and up to 9pm seven days a week on a partial shift rota pattern, but there is no overnight commitment.

DF2/SHOs will travel between hospital sites in Southampton and Winchester to work during normal working hours, but not when on-call. Training opportunities on the rotation are divided between SGH, the ISTC, and the Royal Hampshire County Hospital in Winchester.

Dentoalveolar surgery experience will be gained mainly at the ISTC and Winchester, where DF2/SHOs have the opportunity to be taught and work under close supervision treating patients under local anaesthetic, IV sedation and day case general anaesthetic. In time DF2/SHOs have their own (supervised) LA lists carrying out dentoalveolar surgery and soft tissue biopsies at all 3 hospital sites.

The main department of Oral and Maxillofacial Surgery is located at Southampton General Hospital and provides a broad range of services, including oral medicine, ablative and reconstructive surgery for head and neck cancer, orthognathic and craniofacial surgery (except primary cleft repair), complex cranio-maxillofacial trauma management, salivary gland surgery and skin malignancy. DF2/SHOs can expect to gain hands-on experience in trauma management, particularly soft tissue surgery (eg. suturing lacerations) and fixation of mandibular and zygomatic fractures. They will also assist in complex maxillofacial procedures.

The majority of the DF2/SHO’s time will be spent either in the outpatient department seeing routine or emergency patients, performing/assisting with both elective and emergency patient’s surgery under LA/GA, and the care of in-patients.

The DF2/SHO and FY2 will, in close conjunction with the Specialty Registrars who are medically qualified, provide routine care of in-patients. In-house tuition, both at induction and throughout the year, is provided in areas relevant to the management of emergency and in- patients.

All DF2/SHOs will be allocated an Educational Supervisor, whom they will meet with regularly. There are regular teaching sessions held at the ISTC organised jointly by SGH/ISTC staff, as well as additional sessions held at SGH. There is also a Deanery study day programme which runs throughout the year (held jointly with the DF2/SHOs from QA hospital in Portsmouth). All three hospitals have medical library facilities. There are monthly radiology, pathology, and audit and governance meetings at SGH.

It is a contractual requirement that DF2/SHOs carry out both clinical and medical audits and DF2/SHOs are expected to collect and collate clinical data. There will be protected time provided during the working week for the completion of audit projects under the guidance of the Specialty Registrars/Consultants. During the course of a normal contract DF2/SHOs will have the opportunity to be involved in the publication of at least one scientific paper, and to make a presentation at the regional BDA hospitals’ group meeting which is held twice a year. The post is recognised by the Royal Colleges of Surgeons under MFDS/MJDF regulations, and undertaking these postgraduate qualifications is actively encouraged.

There is an active junior doctor’s mess at SGH with regular social events organised. Southampton is well placed for water based activities on the Solent, and is adjacent to the New Forest which has excellent walking, cycling and horseriding amongst it’s attractions. The city has good leisure facilities for sporting activities, shopping, restaurants and bars, cinemas and theatres. There are good transport links, with London Waterloo 1 ¼ hrs on the train, and direct access onto the M3.

Requirements .

The successful candidate must have full registration or temporary registration with reduced supervision with the GDC, and it is preferable that candidates hold a full driving licence (although there is public transport between hospital sites).

All new staff will be expected to provide written documentation of immunity to hepatitis B and to attend the Occupational Health department as required.
