May 2015 National meeting precis was submitted for award of $10,000 …funding letter sent 5/29; only received 2 letters of recommendation; required documentation not received. Follow up was sent 6/1 and 7/9.

SM15006 Public Health Organization Chicago, IL *Received three letter of recommendation for the group. SM15007 La Conexion Bowling Green, OH *Received three letter of recommendation for the group. SM15008 MPactors Chicago, IL *Received three letter of recommendation for the group.

SM15009 Latino Leadership Organizing (Pink Slip) St. Paul, MN Letters of Recommendation were not included with application.

Please Record in Minutes SM15002 Healthy Growers for Detroit Detroit, MI The Task Force authorized Lisa Leverette to conduct a site visit. Lisa will request a revised budget. SM15002 was rejected. Lisa site visited in May and did not recommend for validation and funding at May’s National electronic meeting. Reason for rejection is needed.

Rescind SM11021 Warriors on Wheels of Metropolitan Detroit Detroit, MI Clara requested quarterly narrative report at May’s National meeting in 2014 and December 2014 requested report by telephone. To date not received. Please rescind for $2,500. Staff: Cynthia White
