What Is Patient Online?

What is Patient Online? Patient Online will give patients the option to make appointments, order repeat prescriptions (‘transactional services’) and view their electronic medical record online, using their computer, tablet or smartphone rather than having to phone or visit their practice. This service is currently available to patients aged 16 years and over.

Online services will complement and not replace the existing ways patient’s access appointments, prescriptions and their records.

Coercion If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time. Access to GP online services Surname First name Date of birth Address

I will contact the practice as soon as possible if I suspect that my account  has been accessed by someone without my agreement 7. If I see information in my record that it not about me, or is inaccurate I will  log out immediately and contact the practice as soon as possible

Please note if you have not registered your log-in details online within 2 weeks of issuing they will be invalid and need re-issuing.

The online booking information provided is legally confidential and privileged and is intended for the named person to whom it is addressed only. Elliott Hall Medical Centre does not accept responsibility for the use or misuse of online booking information or appointments. Any problems or feedback should go to the Practice Manager.

A valid form of photo ID will be required to collect your log-on details, or supply us with a Stamped Addressed Envelope. If you wish for someone else to collect on your behalf please provide written consent.
