Association Screening Checklist

1. USA Hockey / NYSAHA SafeSport Policy mandates that all association volunteers / coaches (over the age of 18) who have regular, routine or frequent access to youth participants within your association must be screened prior to these volunteers having such access. These volunteers include: ◦ Members of the Association Board of Directors ◦ Program Administrators ◦ Coaches ◦ Team Managers ◦ Officials ◦ Locker room monitors ◦ Team drivers ◦ Travel chaperones ◦ Anyone with regular, routine or frequent access to youth participants. 2. These volunteers / coaches must complete Screening and SafeSport training prior to participation in your program. 3. To verify that a volunteer has been screened, go the NYSAHA website at http://www.nysaha.com, select the Screening Policy section from the left margin and click on the link labeled Volunteers List Screened thru Month/Day/Year. The file is sorted in alphabetical order by the volunteer’s last name and provides their respective Report Confirmation Number in the Report Number column. This report can be saved as an excel file to your personal computer and re-sorted to provide custom data. 4. To verify that they have completed USA Hockey’s SafeSport training, go the NYSAHA website at http://www.nysaha.com, select the Screening Policy section from the left margin and click on the link la- beled Volunteers who took SafeSport thru Month/Day/Year. The downloaded file is sorted in alphabetical order by the volunteer’s last name and verifies they completed their training. This file also reports on the individuals screening status with USA Hockey’s registration data. 5. Download a copy of the 2017 / 2018 Association Screening Certification worksheet from the Association Certification Form link.

6. Fill out the Association Screening Certification form with the requested Association information; Name of Association, Association Address, Association Web URL, Association President Name, Date (of certification), Association Presidents (Home) Phone number, Association Presidents Cell Phone Number, The Association Presidents Email Address, the name of the Association’s Registrar, The Association Registrar’s Phone number (cell or home) and the Association Registrar’s Email Address.

7. Then, for each volunteer/coach in your association, list their Name, their corresponding Report Confirmation Number and the Request Date. The Association President, prior to submission, must certify this form as accurate.

8. Complete the form by October 15, 2017, and submit it via email it to [email protected]. You will receive a reply email verifying receipt. A fine of $100 or more will be automatically imposed for submissions after October 15th.

9. Certification forms not received by December 1, 2017, will place the association’s teams in a non compliance period where games played will not count towards their tournament bound eligibility.

10. The association may placed their USA Hockey and NYSAHA membership good standing in jeopardy if this Screening Certification form is still not received by January 2nd of 2018.

11. Association are encouraged to submit updated Association Screening Certification forms whenever new volunteers join the association subsequent to the October 15th filing date.
