Welcome/Introductions: Kris Sherrill of Southern Illinois Healthcare welcomed everyone to the meeting and introductions were given of all in attendance.

Approval of F/WHCC Minutes of 10/20/11: Minutes stand approved.

Announcements/Agency Information Sharing/Handouts: . Salvation Army Shining Stars Program – Individuals across southern Illinois are able to purchase gifts for senior adults in need. Approximately 200 gifts are delivered by case managers from Shawnee Alliance for Seniors.

. Hospice of Southern Illinois Junior Volunteers – will be helping wrap presents at the Illinois Centre Mall on December 15th and 16th and will be taking donations that will go to Hospice of Southern Illinois.

. 2nd Annual Celebrity All American Gala for Hospice of Southern Illinois – January 28, 2012 at the Pavilion of the City of Marion. Pre-Show starts at 5 p.m., red carpet entrance is at 6 p.m. and the celebrity performances start at 7 p.m. There will also be a silent auction and an after party with a DJ. Tables of 8 are available for $500 and tables of 12 are $750. Individual admission is $62.50. This is a formal event. . Marion VA Medical Announcements:

1  Center Caregiver Support – Services are available to individuals that care for Veteran’s. The VA Medical Center is in the process of developing a caregiver counsel and resources. For more information about this caregiver support program, call (618) 997- 5311, ext. 59185.  VA Medical Center Employee Gym – Due to a recent grant received, the VA Medical Center will be getting a full employee gym. VA Medical Center has 1,400 employees that will benefit from this gym and they are also involved in employee wellness with health coaching, etc.  Reach VA Program – for veteran’s with dementia. . The Women’s Center Taste of Chocolate Event– Save the date: February 3, 2012 at the Carbondale Civic Center.

. Franklin-Williamson Bi-County Health Department Flu Clinics – Walk-in flu clinics are held every Wednesday from 8:30 – 11:30 a.m. and 1 – 3:30 p.m. as long as vaccine is available. No appointment is needed. The fee for the influenza vaccine is $30 and pneumonia vaccine is $60. For more information call (618) 993-8111.

. American Cancer Society “Great American Smoke-Out” is scheduled for November 17, 2011.

. Shawnee Health Service Update: Nancy Muzzarelli updated the committee on the following providers:  Alexis Lorinskas, MPAS, PA-C joined Shawnee Health Service in mid-December and specializes in women’s health, obstetrics and gynecology. She is full time and works at Shawnee Women’s Health (618) 457-0465 and at Adolescent Health Center (618) 529- 2621 in Carbondale. She maintains active staff privileges at Memorial Hospital of Carbondale.  Dental Emergency Walk-in Clinic – Beginning January 7, 2012 all Shawnee Health Service patients will have access to this new dental service. The clinic will be open on Saturdays from 9 a.m. – 11:30 a.m. at Illinois Centre Healthcare – Dental located at 3115 Williamson County Parkway in Marion. Walk-in only – no appointments.

Action Team Updates:

. Healthy Lifestyles  Will start planning a Community Leaders Forum for next fall for Franklin and Williamson counties similar to the event that was held this past October for Jackson county. This event will focus on educating decision makers in the communities to use PSE (Policy, System & Environmental) changes to help residents live healthier lives.  START! Walking Paths – still looking for other locations. New path submission forms are located on the Community Health Calendar website at: www.communityhealthcalendar.siuc.edu  HRSA Grant – An application was submitted under HRSA’s Rural Health Outreach program to further work that advances the regional CVD strategic plan developed by the HealthySI Delta Network.

Promotional items available include: water bottles, Community Health Calendar magnets, change/coin purses, and START! . Healthy Seniors  Nancy Muzzarelli and Carletta Hanks will be stepping down as co-chairs of the Healthy Seniors Action Team and the new chair(s) will be announced shortly.  In the early planning stages of the Senior Fun Fest. Save the date of May 11, 2012 as this will be the date of this event. Nancy Doerge of Senior Life Transitions will be in charge of sending out the exhibitor and sponsor registration information.  The next project for the Healthy Seniors Action Team will be a “Toiletry Drive” in February. Fliers will be distributed soon.  Anyone interested in being involved with the Healthy Seniors Action Team is to call Nancy Muzzarelli at Shawnee Health Service at (618) 956-9509 or Carletta Hanks of RIDES Mass Transit District at (618) 253-8761, ext. 312.  The next Healthy Seniors Action Team meeting is scheduled for Friday, December 16, 2011 at 12:00 Noon at Kokopelli Golf Club and Clubhouse Restaurant. . Access To Care  The combined group of the F/WHCC and JCHCC Access To Care Action Team has been meeting the last few months and concentrating their efforts on Oral Health Care issues (29 medical personnel were trained on fluoride/varnishing treatments as well as the enhanced referral program).  Work continues with Southern Illinois Healthcare partnering with Shawnee Health Service and SIU Community Dental Center on Oral Health issues.  BRIERS – Blood Related Illness Enhanced Referral Service – In the process of training primary care physicians and identifying physicians who will take these patients as their primary care physician.  Medical Legal Partnership (MLP) Initiative – Southern Illinois Healthcare is partnering with Land of Lincoln Legal Services on a referral system for patients to get the needed legal services they need. Diane Goffinet, Attorney for Land of Lincoln is scheduled to be our next agency presentation speaker on February 16, 2012.  If interested in joining this action team, please contact Kris Sherrill at (618) 457-5200, ext. 67834 or Nancy Muzzarelli at (618) 956-9509.  The next meeting of the Access to Care Action Team is scheduled for January 11, 2012 @ 2:00 p.m.

Agency Presentation: No agency presentation this month. A holiday celebration was held with lunch provided by Southern Illinois Healthcare. A time of networking and fun followed.

Next Meeting Date/Location/Presentation: Thursday, February 16, 2012 at 12:00 p.m. at the Man- Tra-Con Office in Marion, IL. The agency presentation will be given by Diane Goffinet, Attorney from Land of Lincoln Legal Services.
