ASCLS/CLMA 2010 ANNUAL MEETINGS & EXHIBITS

The American Society for Clinical Laboratory Sciences - Hawaii and the Aloha Chapter of the Clinical Laboratory Management Association would like to invite your organization to participate in our 2010 Annual Meeting and Exhibits. In our efforts to increase attendance and control costs, this year’s meeting will be held on May 18 thru 20 at the Honolulu Country Club. A golf outing is being planned to be held on Monday, May 17 at the beautiful Koolau Golf Club (see attached). Approximately 400 laboratory professionals attended the 2009 Meeting and activities. If you are interested in exhibiting your products, please follow the instructions below. Each exhibitor will receive a table in the main conference room, where all exhibits are located and refreshments will be served. Fill out the enclosed registration form below and mail, email or fax it back as instructed below. You will be able to check the status of exhibit attendees via our website at: www.HawaiiClinLabConference.org that will have both Exhibitors and Program attendee information. Registration forms may be obtained from this website with information being constantly updated. Additional information packets will be provided on this website to all exhibitors with shipping and accessories rental information. Limited hotel rooms will be held at a rate of $129.00+tax per night from May 15 thru 23, (please call the Ala Moana Hotel directly for this rate at (808) 955-4811 and mention the ASCLS/CLMA Meeting).

 Complete form below and indicate the number of table(s) you would like to reserve. We will attempt to honor all requests.

 A fee of $495 will be charged for each 6 ft table (Floor Instruments in excess of the 6 ft space will be charged as an additional table at $495. The fee includes 2 lunches at HCC (additional lunches may be purchased on site). Make checks payable to: ASCLS/CLMA Annual Meeting. We will either email or fax a confirmation a receipt of payments to you. Visa or Master Card will be accepted (please see attached form) and processed by the University of Hawaii Conference Center.
