When enrolling, please send this form (page 1 and 2) by email. By May 30th 2017 please send a signed print copy to: Kaligraf, ul. Grodzka 60/12 A, 31-044 Kraków, Poland, with “Hebdów 2017” on the envelope. Please do not fill in the grey boxes. Boxes marked with an *asterisk are not obligatory. After your application is received, you will be notified of the group you have been assigned to and the payment. If you withdraw by June 2, 2017 we will refund your advance of 50 Euro.
