2017 150 HP and Under OPENS Waiver and Release of Liability

DISCLAIMER CLAUSE:

The Renegade Bass Tournament Association of Eastern Ontario Inc., the executive, shore staff or sponsors will not be deemed responsible for injury, loss or damage incurred by any person either in traveling to the tournament location, before, during or after the tournament event, for any reason including negligence. Competitors should be present to move equipment and open all compartments for Boat Checks. Shore staff will exercise caution when conducting the Boat checks, however, Renegade Bass, its Shore staff and Executives will not be responsible for any accidental damage during the inspection.

AGREEMENT CLAUSE:

I ______having acquainted myself with the rules; I have completed this (Cleary Print Full Name) document and submit it with my entry into the Renegade Bass Open Tournament(s). In signing this application, I hereby agree to be bound by and comply with all Tournament rules and regulations.

I agree to assume all risks while participating in any Renegade Bass fishing tournament or other events including traveling to and from the events. I agree to waive Renegade Bass, the executive, shore staff and sponsors of all responsibility and liability, including negligence, for any damages or injury of any description.

I am expected to follow high standards of sportsmanship, courtesy, safety and conservation. All contestants must adhere to all Provincial, Federal and local laws governing water activities. Any infraction of these fundamental sporting principles may be deemed cause for disqualification.

I agree that any conduct reflecting unfavorably upon efforts to promote safety, sportsmanship, fair competition and compliance with tournament rules shall be grounds for rejecting my application for membership, participation and for disqualification. Any disqualification or suspension from, or other disciplinary action regarding any tournament or fishing organization, shall be grounds for rejecting my application for membership, participation and for disqualification.

I agree, upon the request of the Tournament Director, to submit to a polygraph or stress analysis test, or an interview by a Committee of tournament officials and members. I understand that failure to comply with this request or to provide results deemed satisfactory to the Committee may result in disqualification from one or more tournaments, including the forfeit of remaining tournament fees and/or membership in Renegade Bass. By entering a tournament event, all competitors permit the organizer to publish or otherwise use their name for the purpose of advising other tournament organizers of any disqualification, and the reason for the disqualification.

In case of any conduct not complying with the standards, Renegade Bass shall have the right to refuse any application, or to deny a confirmed application, by returning the entry fee of a previously accepted application, or to disqualify a contestant.

Renegade Bass Tournament Association of Eastern Ontario Inc. 2381 Sheldon Avenue, Ottawa, ON, K2C1K4 THE FUTURE OF LIVE RELEASE TOURNAMENT FISHING I hereby grant Renegade Bass the unconditional right to use my name, voice, video images, photographic likeness and fishing tips and instructions in connection with Renegade Bass, articles and press releases, and I shall not be entitled to receive any royalties or other compensation in connection with such use.

I furthermore guarantee that the boat(s) and tow vehicle(s) used while participating in any Renegade Bass event will be fully insured for damage and liability for all events. I agree to submit a photocopy of the insurance policy providing a basic description of the coverage to Renegade Bass on or before the first tournament of the 2017 season. I will keep this coverage in effect to cover all 2017 tournaments I participate in or provide Renegade Bass with details of changes in coverage or insurers as soon as they occur.

I further understand and agree that Renegade Bass Executive reserves the right to reject my application for any reason.

I hereby waive my rights of privacy or publicity with regard to the programs of the tournament in which I appear and consent to B.A.S.S., LLC's (and its parent, affiliated and subsidiary companies, and those acting under their permission or upon their authority), copyrighting, distributing, televising, publishing, and using in any way the audio and visual portions of any television videotape, film and photographs pictures of me or of interviews, scenes or other sequences in which I may be included, and any reproduction thereof, anywhere at any time through any medium or media for advertising, promotion, trade, television programming or other lawful purpose whatsoever, and I shall not be entitled to receive any royalties or other compensation in connection with such use. ______(initials) I acknowledge and agree that B.A.S.S., LLC (and its parent, affiliated and subsidiary companies, and those acting under their permission or upon their authority) shall have the right to use my personal information to contact me with communications and other advertisements, and may provide such information to any sponsor of B.A.S.S. LLC or its parent, affiliated and subsidiary companies, which sponsor may contact me with communications and other advertisements. ______(initials) The boat(s) that I will be using in the 2017 Renegade Bass Tournaments is/are fully insured, includes at least 1 million dollars in liability coverage and is rigged with a 150 HP or smaller motor. I will provide a copy of the detailed insurance policy(ies) on or before the first tournament of the 2017 season. I acknowledge that failure to provide ANY of the required documentation on or before first tournament will result in my disqualification from the first event or until such time that all of the documents have been submitted ______.
