Letter issued by the Owner of the vessel/ Agent in Original regarding minimum of 3 months supervised Sea Service on the type of Tankers (OIL / CHEMICAL / LIQUEFIED GAS TANKER) with RPSL number and email address or Appropriate Refresher Training .(For Renewal DCE Only) 10. Original Old Level 1 D. C. Endorsement (For Renewal DCE Only) 11. Three Recent Color Photographs (Passport Size 40 X 30 mm) 12.
