Application Instructions DPT Applications *Please read this entire document before you begin filling out any forms. Refer back to this document when you are completing your application. Please follow all instructions in order for your application to be considered. All application materials must be received (not postmarked) by the Graduate College by October 15, 2015 by 5:00 PM CST. 1. Complete the application at ApplyTexas.org. It is very important that you do this as the first step of the entire application process. When completing the ApplyTexas application, SELECT SUMMER I 2016 and it will provide the Physical Therapy major as an option. Submit the $65.00 fee required by the ApplyTexas.org application. The email that you use on your ApplyTexas application will be the email to which all correspondence is sent. After you complete the ApplyTexas application, you will receive an email from Texas State that (1) assigns you an ‘E’ number and (2) provides instructions on how to check on the status of your application at: https://ssb.txstate.edu/prod/twbkwbis.P_WWWLogin

2. Submit official transcripts from all institutions attended. Start this process as early as possible since it often takes time for transcripts to be received.

3. Request your official GRE scores be sent to The Graduate College at Texas State. 4. Complete the Department of Physical Therapy application. Please indicate "NA" or "none" in the space if you cannot provide the information being requested. Also, please label all materials with your name when submitting them. 5. Complete the “Pre-requisite Form” to indicate completion of the pre-requisite course requirements or the equivalent course. If a course is in progress, please indicate the status along with the date of course completion. Also, if you completed or are currently taking a two-course sequence, please indicate the courses and grades for both courses of the two-course sequence. 6. Be sure to use the Texas State Reference Checklist Form as found in the application materials. Completed “Reference Checklist Forms” should be returned to you in a sealed envelope with a signature across the back seal. Submit all 3 forms, unopened, to the Texas State Graduate College. As an alternative, the person providing the reference may email the form to [email protected]. If references are submitting this form electronically, please include applicant’s name, date of birth, and Texas State Student ID number if available, on the form. Reference Checklist Forms should be requested from two (2) physical therapists and one (1) college professor. Your references may submit a recommendation letter in addition to the Reference Checklist Form, however, the Reference Checklist Form must be submitted. It is the applicant’s 2_Application Instructions_DPT 2019 Texas State University Department of Physical Therapy 512-245-8351 (not the references’) responsibility to make sure that all references are received by the application deadline. Please do not submit more than 3 Reference Checklist forms – only 3 forms will be accepted. 7. Submit “Supporting Statements” for the two (2) questions found on the Department of Physical Therapy application form. Responses are limited to one page per statement, double-spaced. Label each page with your name. 8. Review and understand the documents “Expectations for Physical Therapist Students,” and “Technical Standards”. 9. The ApplyTexas application, GRE scores, and all required application documents and fees must be received (not postmarked) by the Graduate College no later than 5:00 PM CST on October 15th, 2015. If not personally delivering the application documents, mail the completed documents to the following address: The Graduate College Texas State University 601 University Drive San Marcos, TX 78666-4605 If you prefer, you may electronically submit the documents listed above to The Graduate College at https://tim.txstate.edu/gadocumentupload/. Individuals submitting recommendation letters for you may submit them electronically to [email protected]. References using this email address will need the applicant’s name, date of birth, and Texas State Student ID number if available. It is in the applicant’s best interest to submit documents electronically whenever possible, as electronically submitted documents are easier to track than those received by regular mail. 10. In-person interviews are scheduled for Monday, November 16, 2015. Applicants will be contacted (via the e-mail used on the ApplyTexas application) for an invitation to the interview no later than November 5, 2015. 11. For those applicants who applied last year and are in the process of reapplying, you must submit the following:  New ApplyTexas Application  Updated official transcripts only if additional courses were completed since the previous application  Updated GRE scores, if applicable  New DPT Application Form  $65.00 ApplyTexas fee  Pre-requisite Form with updated pre-requisite course completion information  New Supporting Statements (2)  You may submit new Reference Checklist Forms. Please provide your name and application ID number or date of birth (D.O.B.) 12. You will be able to access the status of your application by going to the following website: https://ssb.txstate.edu/prod/twbkwbis.P_WWWLogin Please do not contact the Physical Therapy Department to check on the status of your application – they are only able to refer you to this Application Status link. Electronically submitted documents will be received and reflected on your online status very quickly. If submitting documents via the U.S. Postal Service, it will take up to 10 business days from the date materials are received by the Graduate College for the online status to be updated. Therefore, it is recommended that all materials be received by the Graduate College no later than October 1 if you would like the opportunity to confirm that all materials have been received by the Graduate College by the deadline. If materials are not received prior to October 1, the Graduate College cannot guarantee that the online status will be updated prior to the application deadline. Therefore, it is in the applicant’s best interest to submit all documents no later than October 1, to allow enough time for the online status to be updated so that the applicant can identify and submit any required documents prior to the October 15th deadline. 13. Keep your mailing and e-mail address up-to-date by emailing any changes to [email protected]. 14. Please refer to the PT website at www.health.txstate.edu/pt for information or answers to your questions. Please note: In order for your application to be considered complete and to be reviewed by the PT Student Selection Committee, all application documents, reference checklist forms, official transcripts, and official GRE scores must be received (not postmarked) by the Graduate College by the October 15th 5:00 PM CST deadline.
