GammaTech Laptop Disposal Form (State of New Jersey)

In accordance with the New Jersey Department of Environmental Protection, GammaTech Computer Corp. has established the following procedure for all residents of the State of New Jersey to return GammaTech branded notebook computers for disposal.

Please complete the following form and read carefully the attached notes. Once the form is completed, simply Fax to GammaTech at 510-492-0832 or scan and email to: technical_support@ gammatechusa .com, GammaTech will reply with an (RMA) return material authorization number and shipping instructions. If you have any further questions regarding this Recycling Program, please contact us at 800 552-8946 or email to: technical_support@ gammatechusa .com.

Notes: 1. You must enclose a copy of Disposal Form with your laptop(s) in the box and have the RMA number clearly stated on the outside of the shipping box. 2. You are responsible for all confidential data that may be stored on the products. Before you ship the products for recycling, you are responsible for: (a) deleting the data on the hard-disk drives and any other storage devices in the products; (b) backing up or transferring any data prior to deletion; and (c) removing any removable media, such as diskettes, CDs, or PC Cards. Gammatech does not accept liability for lost or confidential data or any software. 3. Gammatech is not responsible for returning or holding any personal belongings and/or information shipped with the laptop. All contents of the shipping box will be disposed upon receiving.
