Please ask me personally as I will only be writing a limited number of recommendations.  Students that have had me for BOTH Calc AB/MACH 5 and Calc BC have priority.  No Packet = No Recommendation  Please be sure to include all requested information.  Failure to do so may result in the recommendation not being done in time.

Forms must be completely filled out by you, including the general parts that are in the teacher section. I will return the form to you if it is not filled in completely. Leave the recommendation sections for me to fill in.  Common App: Make sure to “Assign” me to your colleges. Use the following form (next page) to list the colleges and the due dates for each college/university that will be receiving a recommendation even; if they are all common app. Attach this list to the outside of a 9” X 12” Manilla envelope before turning into me. If this form is not attached, I will return the packet to you.

***Please keep in mind that the more info you give me, the better you recommendation will be. Don’t assume I remember everything about you even though you were a very valuable member of my class .

***If you NEVER talked to me in class or we did not get along, you probably should ask another teacher to write your recommendation. ***If you were caught violating your integrity while in my class, you probably should ask another teacher to write your recommendation.
