The Opening Screen for Microsoft Word Comes up with a Blank Sheet of Paper to Type

1. Click “Free Health Assessment” on the above webpage

2. At the bottom of the new window, click “Accept and Continue”

3. Click “Child”

4. Enter your age and then Orefield Middle School’s zip code. (18069)

5. Select your race, gender and then click “continue”

6. Continue with the assessment answering the questions as accurately as possible. Make sure to answer every question.

 Age, height (feet and inches) and weight must be entered.

7. Click “Calculate”

8. Thoroughly review your report. Copy and paste the report to a word document and save it to your student number.

 Save as “ your grade health assessment”
