Formal and informal interviews will take place on Wednesday 22nd March The informal interview will be with a small group of young people with learning disabilities. Job specification

Person reports to Transition Co-ordinator

Person Supervises -

Salary £22,313 pa pro rata , (£12.26 per hour)

Hours Up to 37 hours, Monday- Friday including evening and occasional weekend shifts.

Purpose of the Job

To empower young people to run their own projects using creative methods of working.

To support young people with disabilities to have more control, involvement and choice as they move from childhood to adulthood.

Management & administration 1. Maintain day to day efficient and cost effective office practice. 2. Communicate effectively within the team and with other teams at the Limes. 3. To contribute to development of policy to ensure best practice. 4. To ensure members have the information, training, support and resources they need to run projects effectively and safely in conjunction with one another. 5. To ensure targets are met in accordance with the Project objectives and produce review/ monitoring reports as required. 6. To become familiar with the policies, services and working practices, including care plans and reviews, of young disabled people, both in the Local Authority Children’s and Adult Services. 7. To produce reports as specified by the Youth and Family Support Co- ordinator and maintain up to date records of a high standard. 8. To ensure that each young person with an individually taliored package receives a quality assured service.

Development 1. To research, promote, develop and carry out activities and projects with consideration to members’ support needs and wishes in line with the project outcomes. 2. To support each member to develop their own person-centred plan. 3. To assist members to build up networks of support, including peer support. 4. To help service users to identify and work on what they need to make a successful transition to adulthood (e.g. housing, employment, independent living skills, independent travel, employment options). 5. To ensure that all members are referred to appropriate agencies for any further advice and support as and when appropriate. 6. To develop members participation in the organisation of projects. 7. To keep abreast of current development in the field of social inclusion.

Co-operation, Liaison and publicity 1. To provide information in an accessible format. 2. To attend staff team meetings or external organisations’ meetings when required by the Youth & Family Support Co-ordinator/Director 3. To provide written and verbal reports for the management of Youth services. 4. To promote opportunities for people with learning disabilities in the community in conjunction with the Youth & Family Support Co-ordinator. 5. To develop links with external organisations and the Local Authority.

General 1. To maintain and develop positive links with those who support members, particularly parents. 2. To support young adults with learning disabilities to understand and sustain professional relationships. 3. To operate within all Limes policies including confidentiality and Equal Opportunities 4. To undertake training as required by the Youth & Family Support Co-ordinator 5. To undertake other duties as required by the Youth & Family Support Co- ordinator within the competence of the post holder and commensurate with their grading
