1. Name, contact information and relationships for 3 references. Professors in the Department other than Joel Grothe can be listed WITH THEIR PRIOR CONSENT. 2. A resume including all theatre and dance experience and training. Include all production work at Lamar and any volunteer/ professional work. Do NOT include theatre/dance work done for a grade. 3. AS SOON AS POSSIBLE, your class/work schedule for Fall. 4. Please include information regarding anything that prevents you from fulfilling the duties of the scholarship/assistantship. 5. Please list any areas of interest and/or areas you are skilled in for an assistantship. Be specific about whether it is interest or skill (please note all requests cannot be accommodated).

Any Falsification of information on this document will result in all scholarships or assistantships that are awarded to you being revoked until further notice.
Endowed scholarships are renewable, but are governed by individual criteria, and are not guaranteed for all years you attend Lamar. They are offered for a maximum of four years.
Assistantships will be reviewed at the end of each semester and will only be renewed if all criteria are met. Qualifications listed below are ELIGIBILITY qualifications; meeting the minimum of these does not guarantee an assistantship.
REQUIREMENTS/QUALIFICATIONS FOR ASSISTANTSHIPS Requirements for endowed scholarships are available in the Department office. Merit awards are given on a semester basis and are not explicitly renewable.
1. In good academic standing with the Department of Theatre & Dance, the College of Fine Arts & Communications and Lamar University.
2. In good standing with the Department of Theatre & Dance regarding all requirements and involvements in your emphasis area.
PLEASE NOTE: PRIOR NOTICE OF EXCEPTION TO ANY OF THESE REQUIREMENTS MUST BE PROVIDED TO MAINTAIN ASSISTANTSHIP.
