Seneca Valley Senior Prom Permission Form May 10, 2014

1. ALL SENIORS are invited to attend the 2014 Senior Prom. There is no after prom. The charge for prom is $100 per couple or $50 per person. Tickets for the event will be on sale before school in the lobby and in the cafeteria during lunches from April 11 to April 17. Only seniors may purchase tickets. Tickets may only be purchased after the permission slip (Form 1) is signed, outside SV guest permission & ID are obtained (Form 2 if applicable), and school obligations are cleared. NOTE: Tickets not purchased during the scheduled sale will cost $110 per couple. There is no guarantee that tickets will be available after the scheduled sale.

2. Prom will be held on Saturday, May 10, 2014. The hours for this year’s prom are 6 pm to 11 pm with the Grand March starting at 5 pm. Grand March will be held in the senior high auditorium. All participants must arrive by 5:30 pm to be part of the Grand March. All prom attendees must ride a bus to the event at The Cathedral. When prom attendees arrive at the campus for Grand March, they will enter the senior high using the entrance closest to the SVTV studio. Parking is available for prom attendees in the lower Senior High Faculty lot.

3. Guests that are not Seneca Valley students need to fill out a guest request form (Form 2) and submit a copy of photo identification. Form 2 can be picked up in the main office and must be turned in when tickets are purchased. Tickets will not be sold until this is done. Guests who are 21 years of age or older must be interviewed by a school administrator. Appointments can be made by calling the senior high school main office at (724) 452-6041 x 1011.

4. All school obligations must be cleared before tickets can be purchased. The signed permission slip (Form 1) must be presented along with cash or check made out to Seneca Valley High School to purchase your prom ticket. April 17, 2014 is the last day to purchase tickets without a penalty. Tickets purchased on April 22nd and April 23rd will cost $60 per ticket to cover late fees. April 23, 2014 is the absolute last day to purchase tickets since exact numbers are due to The Cathedral the next day.

5. Students attending prom must be in attendance at school on Friday, May 9th for at least a half day (10:53 am).

6. Any prom attendee who leaves prom will not be permitted to return, and no changing of clothes will be permitted.

7. All policies and procedures of the Seneca Valley School District will be strictly enforced during the prom.

8. If your son/daughter chooses not be abide by the aforementioned and/or attempts to enter prom without a ticket, he/she will be subject to actions taken by New Castle Police Department.

My son/daughter has my permission to attend the 2014 Seneca Valley Senior Prom on May 10, 2014 at The Cathedral in New Castle. He/She will leave the campus when the buses return from The Cathedral.
