Action: Grant funding obligation action for referenced grant project per NEG agreement for the requested period of performance: October 1, 2013 through September 30, 2015. See Grantee application dated March 14, 2014, which is incorporated by reference.

Additional Special Terms and Conditions: Within 60 days of receipt of this NOO, the Grantee must submit a grant modification request to address the following:

Incremental Funding - The Grantee must submit a revised SF-424 and SF-424A and associated Budget Information Narrative to reflect the initial incremental award of $721,370.

Congressional Districts - The Grantee must revise Item 16b of the SF-424 to add Congressional District 2, as this Congressional District also covers the area affected by the project.

Trade Adjustment Assistance (TAA) Eligible Workers - The Grantee must submit the required TAA certification documents as the planned participants from Avery Dennison Corporation and Dow Jones and Company, Inc., have been certified as eligible for TAA. In addition, the Grantee must ensure that funds awarded under this NEG are not used to pay for training that can be covered by the TAA program for TAA-eligible workers.

Next Modification Request - The Grantee is encouraged to coordinate with its Federal Project Officer for guidance when submitting the next modification request. The Grantee is reminded that it must select the "initial increment" modification type, rather than the "terms and conditions" modification type, in the eNEG System. Selecting a "terms and conditions" modification type is not necessary, as all the appropriate documents to enable the Grantee to address the above terms and conditions will be generated by the eNEG System via the "initial increment" modification type. If the Grantee will be requesting additional changes, another modification type may be required in its next submission.

NOTE: In the Description of Hampden County Dislocation Event Attachment to this modification request, it was stated that 301 worker surveys were conducted, whereas the number of surveys shown across the Employer Data Forms contained in the application adds to 403 surveys. As this attachment was submitted specifically to justify this initial request and is not needed with future modification requests, no further action is required of the Grantee to address this issue; it is simply noted for the record.

In addition, at the time this application was submitted, an eNEG System error created four narratives in the Employer/Events Narrative Statements, which contain information that appears to be associated with employers the Grantee had submitted Employer Data Forms (EDFs) for in previous versions of this application, but were not a part of the final application. No further action is required of the Grantee to address this issue; it is simply noted for the record.
