Dispensary Manager

The Royal Infirmary of Edinburgh is a large teaching hospital with an established reputation for offering the best practice across a wide variety of specialist services. The pharmacy delivers high quality dispensing services to in-patients and out-patients including clinical trials. In 2016, the pharmacy department will begin a programme of enabling works to prepare for the new Royal Hospital for Sick Children and Department of Clinical Neurosciences due to open on the Little France Campus in September 2017 which includes the renovation of the dispensary, extension of the patient waiting room and implementation of automation amongst other alterations.

We are looking for a motivated pharmacy technician to manage the dispensary service throughout this period of change. Candidates must be registered with the General Pharmaceutical Council; be a designated checking technician and hold the HNC in Pharmaceutical Sciences or equivalent. 2. JOB PURPOSE To manage, co-ordinate and develop dispensary services to meet the needs of patients in the Royal Infirmary of Edinburgh and Princess Alexandra Eye Pavillion in accordance with identified divisional priorities.

To support the activities of the Lauriston Building pharmacy service.

To develop and co-ordinate an efficient one stop dispensing system within a multidisciplinary team. One stop dispensing system refers to the practise of combining inpatient and discharge medicines into a single labelled supply for discharge.

To contribute to the strategic and operational planning of pharmacy services and to help ensure the highest standards apply to all aspects of medicine supply. To optimise the role of electronic systems in supporting the delivery of a highly efficient dispensing service.

5. ROLE OF DEPARTMENT

The aim of the Pharmacy Service is to assure quality of patient care in the provision of treatment with medicines. To this end the objectives are:-

(i) to provide pharmaceutical care to individual patients by meeting their particular needs while maximising efficiency in the use of resources. (ii) to provide medicines through systems of quality control which ensure safe, effective and economic use.

Dispensing services supports the achievement of the above objectives through the provision of dispensed medicines that comply within legal, national and local guidance.

The Pharmacy Department has a major role in the teaching and professional development of Pharmacists and Pharmacy Technicians and other health care staff.

The Pharmacy Department participates in and supports the organisation’s clinical effectiveness programme through participation in clinical audit and research and development.

6. KEY RESULT AREAS

(i) Main duties and responsibilities ( 90% of activity )

Management

To be responsible for the management and lead the strategic development of Dispensing activities, to ensure medicines are dispensed accurately to individual patients and supplied efficiently, from dispensaries and ward based one stop dispensing services. To work within the agreed national and local strategies and statutory requirements for medicine supply, to ensure that required dispensed medicines are available to the patient at the time they are needed. To ensure that the supervision of day-to-day activities in the dispensary is provided by suitably trained personnel and that the correct skill mix is maintained To liaise with ward based staff in reviewing and developing dispensing services to ensure best use of pharmacy resources. To undertake the change management required to modernise the dispensary services with reference to the departments’ strategic plan. To collate and report on all dispensary workload activity data to influence decisions on service development To facilitate effective communication systems for dispensary staff. To participate in working groups when required as appropriate. To liase with the clinical pharmacy teams to provide adequate pharmacist cover for dispensary activities, prioritising and directing as required to ensure service delivery is maintained at optimum. To plan and co-ordinate technical resources to provide adequate cover for the ward service activity, prioritising and directing as required to ensure service delivery is maintained at optimum. To participate in audit (internal and external) of Dispensing Services and action as required. Undertake a systematic approach to good dispensing practice. To manage and develop the patient pack over-labelling system to support one stop dispensing.

Professional Activity To accurately dispense discharge, outpatient, inpatient and clinical trial and prescriptions as required, so that patients receive the correct medicines. To accuracy check and final release of dispensed medicines, inpatient supplies and patient pack overlabelling to ensure that patients receive correct medicines

Human Resources

To undertake sickness absence and disciplinary reviews in conjunction with Human Resources in accordance with current NHS Lothian policies and procedures. To manage resolution of staff issues, for example absence management or conflict when required. To identify and manage under-performance by implementing and supporting corrective remedial action. To co-ordinate and participate in the recruitment and selection of relevant pharmacist, technical and pharmacy support staff.

Education and training

To participate in and co-ordinate and maintain a system of performance appraisal in accordance with the principles of continuing personal development and Performance Planning Systems. To review, participate in and encourage continuing professional development for dispensary staff to ensure personal knowledge base is maintained and improved as required for registration with the General Pharmaceutical Council To co-ordinate and supply in-service education and training to pharmacists, pre-registration pharmacists, technicians, and support staff. To co-ordinate and supply in-service education and supervision of locum, vocational and seasonal staff within the department. To liaise with the clinical trials team to support training for pharmacy staff on aspects of handling of clinical trials and provide an internal and external pharmacy contact for help and advice regarding clinical trials To participate in the teaching, training and presentation on external and internal courses and conferences.

Quality

To co-ordinate key performance indicators. Review, monitor and maintain quality and performance standards and risk management strategies for the dispensary functions. To co-ordinate in the development, maintenance and monitoring of standard operating procedures within the dispensing section of the department to achieve improvements in service quality and to conform with the Quality Policy (BSEN ISO 9001:2000) To review, investigate and report on customer complaints. To report, investigate incidents and near misses and to implement preventative and corrective action in accordance with risk management policies. Take the dispensary lead for external audits by BSI and internal audits by the Quality Assurance department. Take action on any non-conformance or observations reported after these audits. To prepare and review all documentation relevant to extemporaneous dispensing and to establish and maintain a document control system

Health and Safety

To co-ordinate essential health and safety reviews and assessments. To co-ordinate environmental monitoring and maintenance of essential equipment. To attend Fire lectures and Life support training annually: Manual handling, Health and Safety and COSHH assessment management as required.

Information technology

To work with the Head of Procurement and Distribution, the Information Services Department and JAC Pharmacy System Manager to develop and maintain the pharmacy computer database to ensure that it is secure, accurate and fit for purpose. To utilise, introduce, develop and implement electronic solutions to optimise the efficiency of the dispensing service.

Physical resources

Ensure facilities and equipment are maintained to the appropriate standard and optimum use if made of them. To manage medicine stocks held in the dispensaries and co-ordinate stock management controls which include weekly stock value and regular stock holding reviews, annual, rolling and high value stock takes.

Medicines Management

Collaborate with Medicine Management Pharmacy staff to assist with implementation and promote awareness of evidence based prescribing within the Lothian Joint Formulary, guidelines and prescribing algorithms. Interpret data in order to prepare reports to support the analysis and decision making process within Lothian for the introduction of new medicines

(ii) Other duties ( 10% of activity)

To participate in deciding and implementing departmental policies and procedures in liaison with other section managers and the Site lead Pharmacist. To be jointly responsible with other staff members for security of pharmaceuticals, security of keys and use of the alarm system. To participate in research in to appropriate areas of practice as required. To supervise project work Participate in departmental rotas including evenings, weekends and public holidays. 7a. EQUIPMENT AND MACHINERY To use JAC pharmacy stock control and dispensing system for labels, management reports, import new products, stock management and password maintenance i.e. undertake superuser role To use Microsoft Office suite for e-mail, Internet enquires (CPD), word processing for documentation (management information statistics) PowerPoint (presentations of management information, CPD, training and presentation / teaching) and Access databases as required. To use Datix risk management software in accordance with divisional policies. To use unidirectional airflow cabinet. To use PeCos for placing, authorising and receipt of orders for supplies. To use other electronic systems to process worksheets and labels for overlabelled items.

7b. SYSTEMS To comply with the Royal Pharmaceutical Society of Great Britain’s ‘Medicines, Ethics and Practice’

To ensure compliance of ICH GCP (International Conference of Harmonisation Good Clinical Practice) in relation to clinical trials To ensure compliance for the provision of clinical trial medicines under the Medicines for Human Use (Clinical Trials) Regulations 2004 To comply with the Quality Policy of the Hospital Pharmacy Service. To ensure that the requirements of the Quality System (BS EN ISO 9001:2000) are met. To work with the Head of Procurement and Distribution, the Information Services Department and the JAc Pharmacy System Manager to develop and maintain the pharmacy computer database to ensure that it is secure, accurate and fit for purpose.

8. ASSIGNMENT AND REVIEW OF WORK

The post holder is a member of the senior staff of the department representing the dispensing service at departmental operational and risk management meetings Operates within strategic and personal objectives defined policies and procedures. Undertakes continuing professional development. Quarterly service review meetings with the Site Lead Pharmacist. The Site Lead Pharmacist in accordance with the principles of the performance appraisal system carries out review of performance and objective setting.

9. DECISIONS AND JUDGEMENTS

Directly responsible for management, co-ordination and development of dispensing services within defined procedures and standards of work. Responsible for identifying and resolving deficiencies in the service within the scope of work or report them to the Site Lead Pharmacist. Contributes to the strategic and operational planning of pharmacy services. The post-holder has a high degree of autonomy and is responsible for the effective use of own time. Plans and organises own workload and the workload of the dispensary staff. Required to anticipate and assess situations and circumstances to determine the appropriate course of action when problems arise, for example dealing with medication incident reports and ensuring delivery of service is not interrupted due to staffing deficiencies e.g. business continuity planning Required to act independently within guidelines and decide when it is necessary to refer to line manager. 10. 11. COMMUNICATIONS AND RELATIONSHIPS

Liases with Section Managers, Royal Infirmary of Edinburgh Clinical Pharmacists Clinicians and nurses NHS Lothian Dispensary Managers Senior Technicians, Royal Infirmary of Edinburgh

The post holder will have links with Clinical Lead Pharmacists and ward and clinic managers to ensure that the level of service is appropriate and responsive. The post holder is required to have a high degree of co-ordination with section managers and divisional section managers. Communicates effectively with all pharmacy and non-pharmacy staff verbally and in the preparation of written reports. Provides information advice and counselling on medicines to patients and their carers' to support the safe and effective use of medicines. Communicates directly to patients and their carer’s and fellow health professionals, in response to medication incidents reports. To provide and receive highly complex, sensitive and contentious information whilst maintaining a position of integrity and confidentiality when dealing with patient related issues Employ de-escalation techniques to manage and resolve issues of customer expectations. Makes presentations to meet in service training and external training requirements.

Participates in the following Groups:

Weekly departmental operations meeting to contribute to strategic and operational planning of pharmacy services Participate in the Pharmacy Health and Safety Committee to promote essential health and safety reviews and assessment. Participate in the JAC User Group meetings to develop and maintain the pharmacy database. Core member of the RIE Pharmacy Senior Technicians Group.

12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB

High degree of concentration, precision and accuracy for dispensing and final prescription checking, often with strict time constraints and interruptions. Lengthy periods of standing during dispensing and final prescription checking process. Manual handling related to stock and use of equipment. General keyboard skills, can involve lengthy periods of VDU operation. Potential exposure to toxic pharmaceutical materials. Resolution of staff issues, for example absence management, dealing with conflict or conveying unwelcome news may be emotionally demanding. Resolution of customer complaint may be emotionally demanding, including the occasional requirement to deal with aggressive behaviour from patients or staff. Supporting staff that have made a dispensing error, which may have resulted in patient harm, help them cope with having made the mistake and encouraging reflection of their performance at an emotive time may be emotionally demanding. 13.
