Brandon Holm, age two years ten months, is an adorable boy who has been receiving speech and language services two times per week for forty-five minute sessions. Therapy takes place in the family room of the home. Mrs. Holm is at home for all of Brandon’s therapy, and she listens from an adjacent room. We discuss his progress at the conclusion of each session. Brandon enjoys many of the activities presented to him, and continues to make nice progress toward his goals. As of January 3, 2011, Brandon is enrolled in ACDS, where he goes three afternoons each week for a socialization class. In addition, he is receiving one time per week of PROMPT therapy.

Therapy has been focusing on improving Brandon’s receptive and expressive language skills, his oromotor skills, and his pragmatic skills. Brandon is showing progress in his ability to follow simple directions. He is able to complete simple puzzles, and does well with nesting cups. Brandon likes to play with cars and recently he watches “Blues Clues” on television. He has a limited ability to attend to any activity that is not of his choosing. During these times, Brandon tends to become upset easily and will throw himself on the floor, pushing his head into objects. He requires redirection toward a more positive method of behavior, so he does not hurt himself. Brandon’s ability to point to pictures is inconsistent, and often requires hand-over-hand assistance. He does respond “no” to yes/no questions, although not always with the desired meaning.

Brandon is working on increasing his speech sound repertoire. He is showing a nice improvement in overall verbalizations. Whereas before he was generally quiet during play, Brandon is now displaying more times of babbling type speech. He has a limited core vocabulary, which consists of about 25 word approximations, such as /ka/ for “car” and /koo/ for “Blues Clues”. While Brandon gains a word approximation, he tends to lose another word he had used in the past. Therapy continues to work on increasing consonant-vowel combinations. In addition, he is working on lip pursing and retracting exercises, as well as tongue plays to increase his ability to produce more variety of sound combinations. Speech and language skills have been assessed using the Rossetti Infant Toddler Scale. For Language Comprehension, Brandon received an age performance score of 18 to 21 months. He is able to identify four body parts and clothing items on himself, understand the commands “come here” and “sit down”, and choose five familiar objects upon request. He was unable to follow a three step unrelated command, identify parts of an object, and respond to wh- questions. These are expected of a child who is Brandon’s age. For Language Expression, Brandon received an age performance score of 18 to 21 months. He is able to use single word approximations more frequently, increase his use of sentence like intonational patterns, and imitate environmental noises. He was unable to relate recent experiences through verbalization, use verb forms, and express physical states. These are expected of a child who is Brandon’s age.

It should be noted that at the time of the writing of this report, Brandon is being evaluated for services through the CPSE program of his home school district of Hicksville.

It is recommended that Mr. and Mrs. Holm continue to use simple language with Brandon throughout the day. It is also suggested that they provide him with the opportunity to interact with his peers, both in a structured setting, such as his socialization class, and in an unstructured setting, such as the playground or a play date.
