SUBJECT: Revised Facts About the Child Support Program for Foster Care Recipients (CS- 333.2), Facts About the Child Support Program for Medicaid Only and K-CHIP Recipients (Form CS-333.3), and Procedural Instructions
The CS-333.2, CS-333.3 and their procedural instructions have been updated to revise the language and format. This has been done to be consistent with other recently revised child support forms and procedural instructions.
The CS-333.2 has been added to the Division of Child Support’s Internet Web site for informational purposes only. This form is provided to the Department of Protection and Permanency (DPP) with a one-time notification of DPP’s rights and responsibilities in regard to receiving child support services for Foster Care (FC) cases.
The CS-333.3 has been updated to include the following statement regarding the lack of a client/attorney relationship: “I understand and acknowledge that any CFC attorney to whom I am referred will assist me, but is not my personal attorney and will not represent me in any legal matter because of his or her obligation to provide legal services to and for CFC and the Commonwealth of Kentucky.”
THIS TRANSMITTAL LETTER IS TO BE SAVED AND FILED FOR FUTURE REFERENCE AS IT EXPLAINS CHANGES THAT HAVE BEEN MADE TO THE KENTUCKY CHILD SUPPORT FORMS.
The Prosecutors’ Handbook, Manual of Forms, and Child Support Forms are to be updated as shown below and on page two.
