This contract specifies the number of practicum hours required for the student’s learning experience.

For the Masters of Humans Services program a minimum of 125 practicum hours are required. These hours are to be completed in a time framework agreeable to the supervisor and student. All but 10 practicum hours need to be completed prior to enrolling in HS 599 Practicum.

If the practicum hours cannot be completed, a conference between course faculty, supervisor, and student will occur in order to successfully resolve the issue. Documentation of practicum hours is the responsibility of the student and is to be signed and dated by the supervisor.

Upon completion of the required practicum experience hours, the BSN Clinical Practicum Experience Documentation is attached to this contract and filed with the RN to BSN Nursing Program.

The specific learning objectives for this clinical practicum experience are found in the course syllabus.

The student will provide personal learning experiences objectives on the first day of the practicum experience.

Masters of Human Services Student Evaluation Reflection Guide Things to consider as you evaluate your supervisor:

The Supervisor as a role-model; 1. How many times did your supervisor meet with you, and did he/she allow you an exchange of feedback? 2. Did you feel there was open communication and trust between you and your supervisor? 3. Were you offered support? How?
