IN SCHOOL DAYTIME USE REQUEST FORM

This form is to request the use of the BRSD Sports Complex for week day, daytime use ONLY (when school is in session). The purpose is to avoid scheduling conflicts, inform facility management, and to responsibly fulfill your requests. Complete, copy and interoffice this form to Ricardo Pina, Director of District Safety & Security and Sports Complex Manager, at least 15 days before the date of the use request. Mr. Pina will notify you and your Principal by email or phone within 48 hours the status of your request. Signature of Pat Lynch, Principal or Joe Sprague, Athletic Coordinator, or Mr. Pina is required for any request. Thank you!
