- Most Original Fundraiser2012-2013 Annual Service Program Report Form A. DESCRIPTION OF FUNDRAISER: 1. Explain only definite fundraisers. Individual contributions are not reportable unless they are the result of committee planning and arrangement. 2. Explain your fundraiser(s), depending on the category, using the following headings: (a) DESCRIPTION - Describe the item(s) or service(s) offered for the fundraiser. (b) TOTAL RAISED – Total amount raised. (c) TOTAL NET – Total amount netted after costs.
B. REPORTING - The narrative portion must be completely typewritten, doubled-spaced, one-sided, and 8-1/2”x 11” size sheets should be used. Supportive materials (photographs, newspaper articles, videos, etc.) may be submitted with the report. Financial details need to be included: 1. SINGLE FUNDRAISER - Give the name of the fundraiser. Explain by DESCRIPTION, TOTAL RAISED, AND TOTAL NET. Not to exceed three (3) double-spaced typewritten pages. 2. MULITPLE FUNDRAISERS - Name and number (1, 2, 3, etc.) EACH of the fundraisers. Explain by DESCRIPTION, TOTAL RAISED, AND TOTAL NET. Not to exceed one paragraph per fundraiser. 3. MOST ORIGINAL FUNDRAISER – Give the name of the fundraiser. Explain by DESCRIPTION. Not to exceed three (3) double-spaced typewritten pages. Photos, newspaper clippings, etc. may be included with your fundraiser description.
C. AWARDS - One (1) overall WINNER and up to two (2) Honorable Mentions in each of the following 3 categories: (1) Single Fundraiser Category (awarded in 4 club size divisions) (2) Multiple Fundraisers Category (awarded in 4 club size divisions) (3) Most Original Fundraiser (all clubs competing for district-wide honors)
