People Serving People Volunteer/Intern Agreement

1. Data Privacy: Identifying information about PSP clients (including names) must not be revealed, now or in the future. A volunteer must never acknowledge that any person has received services from PSP. 2. Fraternization: The following is considered fraternization and must not take place between volunteers and clients. Volunteers must be easily identifiable by wearing a nametag at all times in the facility. A client is any person who has received services from PSP within the past 12 months: A. Development of any type of personal involvement with a client. B. Going out with or meeting with clients other than in the normal course of job performance. C. Purchasing anything from a client, making any kind of trade with a client, selling anything to a client, or assisting a client in the sale of anything. D. Transporting a client. 3. Respect for Clients: It is our expectation that all PSP clients will be treated with respect by all paid and volunteer staff at all times. Each client is entitled to respect regardless of their behavior. If a client is treating volunteer staff or other clients in a disrespectful or inappropriate manner, a supervisor should be contacted immediately. If this is not possible, please notify front desk staff. Activity Release: Volunteers for PSP accept sole responsibility for any injury they may incur during the time he/she is working as a volunteer. PSP and their employees/agents are hereby released from any and all claims or cause of action arising from any accident or injury that may occur during volunteering. This release shall not operate to release PSP from any claims or cause of action which arise from accident or injury which is due to the negligence of PSP. By signing below I understand and agree to the activity release, and give permission to conduct a background check at the Department of Human Services/MN Bureau of Criminal Apprehension. In exchange for PSP accepting me as a volunteer, providing me with experience and training, I agree that PSP is allowed to use photographs or video that may be taken of me as a volunteer at PSP. I agree to act according to the conduct outlined, and adhere to appropriate behavior relating to issues of Data Privacy, Fraternization, and Respect for Clients. I also understand that PSP has the right to place or not to place me(or my group) in a volunteer/Intern Position.
