Practice Manager: The Practice Manager is responsible for the efficient administration of the Practice. Our aim is to give you the highest possible standard of care. Any complaint or problem will be dealt with as swiftly as possible

Reception staff: Reception staff will help you with your booking of appointments and general enquiries. They cannot give medical advice

Training Practice: We are an approved training practice for specialist General Practice training. Trainees are fully qualified doctors who have completed a number of years working as hospital doctors. GP Registrars usually spend a minimum of 12 months attached to the practice and Foundation Year 2 Doctors usually spend a minimum of 4 months. From time to time one of our Doctors may sit in with the trainee as an observer for educational purposes

Patient Information: Our aim is to provide you with an efficient, courteous and confidential service. Patients are responsible for treating our staff and the services we provide with respect.

Data Protection: This practice has a confidentiality policy in place. To ensure you are provided with the appropriate health care there will be times when you’re personal and medical information will need to be shared with other NHS health professionals (e.g. hospitals or district nurses). For further details please ask at reception

Patient Participation Group: Would you like to have a say about services provided at Daleham Gardens Surgery? By providing your contact details we can get in contact with you every now and again to ask your opinion regarding the services we provide. Please speak to reception for a contact form.

Named GP: All new and current patients are allocated a Named GP who is responsible for your overall care at the practice; however you are welcome to see any GP within in the practice. Patients Named GP is Dr G Wong. If you have a preference as to which GP you are assigned to, please inform a member of staff and we will make reasonable efforts to accommodate this request

Registration Process: All persons living in our catchment area who wish to register will need an appointment with our nurse for a New Patient Health Check. We will register anyone who is entitled to NHS treatment. Before registering please ask about our catchment area to establish whether you live within our boundary. To register, you will need to complete two forms: a GMS1 form and a New Patient Health Questionnaire (Adult & Child). You are welcome to collect these forms from the Practice or print them from our Website. Please return the completed forms in person and we will arrange a New Patient Health Check. The following ID and documents must be supplied with your GMS1 form and Health Questionnaire to complete the registration process: - Photo identity - Proof of Address dated in the last 3 months (utility bill, bank statement, rent book)

Booking an appointment: Please contact us on 0203 112 0800 or visit the Surgery and speak to reception to book an appointment. You have the right to book an appointment with the doctor of your choice or the practice nurse. Punctuality is appreciated. If you cannot keep your appointment please cancel so that we can offer the appointment to someone else. If you wish to speak to a doctor or nurse please contact us during our opening hours and request a telephone call

Patient Online Access: Patient Access lets you use the online services for our practice. This includes booking appointments, ordering repeat medication and access to your medical record (Medication & Allergies). Please speak to reception if you would like to sign up to Patient Online Access.

Urgent Appointment: If you have an urgent medical problem please contact the surgery as soon as possible and we will offer you an emergency appointment

Home Visits: If you require a home visit please contact the surgery

Seeing a Doctor when we are closed: If you require medical assistance between the hours of 12noon – 2.30pm Monday, Tuesday, Wednesday & Friday call CAREUK: on 0207 388 5800. Please notify us immediately of a change of address or any other details. A form is available at reception to amend your medical records

Test Results: Please contact the surgery for test results on Monday, Wednesday and Friday between 2.30pm and 5.30pm

Repeat Prescriptions: Prescriptions can be ordered in person (using an order form), by post, fax, online or by your local pharmacy. Prescriptions can be collected or posted (if we are supplied with a Self- Addressed Envelope) or they can be sent directly to your chosen pharmacy. Repeat prescriptions will be processed within 48hrs. If your prescription will be collected by someone on your behalf we will need your written or verbal authorisation to do so. Many local pharmacies can order repeat prescriptions on behalf of our patients, contact your pharmacy to see if they provide this service. We are also able to send your repeat prescriptions electronically to a nominated pharmacy of your choice; speak to the pharmacist to sign up to this service. For ordering repeat prescriptions online please speak to reception

Travel Immunisations: Please book an appointment with our nurse at least four weeks before you travel. Some vaccines are available on the NHS. We are not a Yellow Fever vaccination centre.

Practice Nurse: Our Practice Nurse is available for the following: Blood pressure checks, Contraceptive advice, Immunisations & Vaccinations, Cervical smears, Smoking Cessation advice, Dietary advice, Ear syringing, Dressings and removal of sutures. Patients with the following conditions are sent invitations to attend a yearly check: Diabetes, Asthma, Coronary Heart Disease, Chronic Obstructive Pulmonary Disease and Hypertension.

Maternity Services: Pre-conceptual advice is available. We offer shared care with our local hospitals for ante-natal checks. Our doctors also offer post-natal care.

Health Visitors: The health visitor attached to this practice is based at Gospel Oak Health Centre and can be contacted on 0203 317 5825. The health visitor gives advice on child immunisations and offers developmental checks
