Volunteers Needed for Diabetes Week 2015

November 8th through November 14th

The Diabetes Coalition of Palm Beach County is a voluntary alliance of healthcare organizations and community partners working together to prevent diabetes and improve the lives of people living with the disease as well as those who are touched by it through Education, Awareness, Quality Services, Management, and Advocacy.

Annually since 2012, the Diabetes Coalition has organized screening assessment sites during Diabetes Week throughout Palm Beach County (including the Glades) to conduct one-on- one Diabetes Risk Assessments using the American Diabetes Association (ADA ) Risk Assessment tool and Glucose Screenings. The Risk Assessment and Glucose Screening help participants determine their risk level for type 2 Diabetes. If found to be at risk, residents are referred to Palm Beach County’s network of health care programs. All individuals screened will receive a free tote bag and resources.

During Diabetes Week, volunteers, physicians, nurses, students and others are needed to assist with conducting these one-on-one interview assessments and glucose screenings at various sites and locations throughout Palm Beach County. Volunteers are also needed for sites where a large number of non-English speaking residents are expected. English, Spanish and Creole-speaking volunteers are needed.

We also need volunteers to help us with pre-event preparation, set-up, and post-event assessment and analyses activities which include:

. Preparing screening site packets and “filling goodie bags” to be given to each screening participant during the week of November 8th-14th. . Pre-event set-up at the Palm Beach Outlets Mall from 2:00 pm to 4:30 pm for Diabetes Night Out on Thursday, November 12th from 5:00 pm-8:00 pm which is hosted by the Diabetes Coalition, in collaboration with Palm Healthcare Foundation and the Palm Beach Outlets. . Post-event clean up – after our Diabetes Night Out celebration at the Palm Beach Outlets from 8:00 pm – 9:00 pm. . Volunteers to work in collaboration with the Diabetes Coalition’s Resource Data Committee to collect, tabulate, analyze, and present data gathered from Diabetes Week. All volunteers assisting with Risk Assessment Tests and Glucose Screenings will be required to complete volunteer training which is offered via the internet on the Diabetes Coalition of Palm Beach County’s website: www.diabetescoalitionpbc.org. To volunteer, please complete the Volunteer Registration Form. Refer to the 2015 Diabetes Week Screening Location Chart that lists dates, times and locations that volunteers and or translators are needed. Please note: We recognize that you may not be able to volunteer for the entire time of the screening event, but we ask that you commit to as much time as possible.

All volunteers are asked to report to the site location 30 minutes prior to the actual screening event to allow for set-up and if relieving another volunteer for a smooth transition.

Diabetes Week 2015 Post-Event Data Collection and Analyses Activities I would be interested in helping the Diabetes Coalition Resource Committee in post-event data activities that include tabulate, analyze, and present data gathered from Diabetes Week. (Those interested, will be contacted after Diabetes Week.) ______Through your participation and our collaborative efforts, the Diabetes Coalition of Palm Beach County will achieve its goal of preventing diabetes and improving the lives of people living with this disease as well as those touched by it through education, awareness, quality services, management and advocacy.
