h. Provide information on how students are screened to assess their ability to succeed in the alternative format or otherwise trained or prepared to succeed in taking course work in this format.

i. Provide information on the training and support for the development and instruction of courses delivered in the alternative format.

j. Provide information on the assessment that is conducted to determine the effectiveness of the alternative format. Attach any proposed assessment instrument(s) you plan to use at the end of this form.

k. Additionally, for totally online courses, provide the following information:
