This Addendum forms part of and modifies Bid Documents dated July 22, 2016, with amendments and additions noted below.

A second walk through has been scheduled for Tuesday, August 9 th at 1PM. Operations Center Building (OCB), Room 1.160H

Be sure to use the new pricing and delivery section, entitled SECTION 6 – RESPONDENT’S BASE PRICING AND DELIVERY BID - REVISION 2, and incorporated below.

The secured area on the 2nd floor is now being removed from the base bid and is to be priced as Alternate 7 and Alternate 8 for the alternates.

We have added an area in the Annex Building which will be part of the base scope of work. This location is for mail services and has sorting machine and other items that would potentially need to be moved.

This project will follow the University of Texas System OFPC Prevailing Wage Determination included with Exhibit C – Special Conditions.

A full time superintendent will be required.

All abatement or other hazardous material removal will be performed by UTHealth.

The start date will be determined after award.

All corridor work will need to be performed in a manner that allows access to occupied spaces at all times.

The existing ceramic tile floor and base in the main corridors of level 1 will be demolished.

The existing ceramic tile in the restrooms (floors and walls) will be demolished.

All granite tiles in the corridor will remain.

All wood or stone wall base will be salvaged for re-use.

ADDENDUM 2 ITB 744-B1636 Page 1 of 8 Floor leveling will not be required, however; you will need to float as appropriate to provide smooth surfaces free of trip hazards.

Toilet accessories to be salvaged and re-used (paper towel & soap dispenser, mirrors, etc.). Feminine product dispensers will be removed.

Remove and re-use all corner guards.

Chair rail in corridor will remain as is (no stain, paint or demo).

All corridors will have the base color applied to the walls above the chair rail with the accent color below the chair rails.

Conference room wood millwork will remain.

The pricing guide details in the base bid to re-laminate the millwork in the break areas and kitchens with an alternate to demo and replace.

Conference room acoustical wall panels will be removed, patched and painted.

Stairwell scope of work only includes the following as applicable: paint, light fixtures, diffusers, carpet or VCT to replace existing conditions (no rubber flooring or tread replacement).

The Rio Grande Room located on the 2nd floor of OCB will not need carpet. It is room number 2.200.

Out intension is to have all plumbing fixtures replaced and matched with existing carriers. See plumbing specification 22 40 00.

All existing drinking fountains will remain.

Supply grilles: Titus Model Number TMS/TMS-AA Frame Type 3 Full Face (Lay-In), Contractor will verify Nominal Round Duct Size B or Nailor Model #RNS3 Type L Lay-in T-Bar, Contractor will verify Nominal Round Duct Diameter

Return grilles: Titus or Nailor 24” x 24” perforated return air grille.

All can lights in conference rooms and elevator lobbies will remain.

LED specified light is fully adjustable so no need for any additional dimmable lighting in conference rooms.

2 x 4 LED light: CREE ZR24-40L-40K-CMA w/ Smartcase; CM-CWC-1 Smartcast Controle Mod.

2 x 2 LED light: CREE ZR22-32L-40K-CMA w/ Smartcase; CM-CWC-1 Smartcast Controle Mod.

Vertical Laminate: Wilsonart D315-60 Platinum

There are no metal ceilings or specialty suspension trim on this project.

Attic stock required for: carpet, ceiling tile, paint, ceramic tile for restrooms and corrodors

Corridor ceramic tile: Daltile Unity P405 12 x 24

Breakroom ceramic tile: Daltile Unity P405 12 x 24

Alternate 4 is a complete replacement of the ceiling grid due to the upgrade to 2 x 2 LED lights. This will alleviate any need to move sprinkler heads or smoke devices.

Having carefully examined the Project Requirements, the General Conditions, the Plans and Specifications and any Addenda to the Plans and Specifications as prepared by the University of Texas Health Science Center at Houston (the Owner of this Project), as well as the premises and all conditions affecting the work, the undersigned promises to furnish all equipment, labor, materials, supervision, services, and required bonding to complete the entire work in complete accordance with the above document for the following firm, fixed prices. University’s standard payment terms for services are “Net 30 days.” Proposer agrees that University will be entitled to withhold five percent (5%) of the total payment due under the Agreement until after University’s acceptance of the final work product. Time is of the essence in the performance of Contractor’s duties. Failure of the Contractor to notify UTHealth sufficiently in advance of inability to complete within the delivery schedule, shall grant UTHealth the option of imposing liquidated damages in the amount of fifteen hundred dollars ($1,500.00) per calendar day. Notwithstanding the foregoing, UTHealth shall have no obligation to accept late performance or waive timely performance by Contractor.
