CORE Mini-Grant Application 2009

Critical Dates and Selection Process

 The call for proposals will be disseminated beginning February 23, 2009.  Applications are due March 20, 2009.  Award notifications will be released April 1, 2009.

Review Criteria Applications will be reviewed by a committee who will rate each application on a100-point system and rank each based on selection criteria that include:  Completeness of team, including administrative support and parent involvement  Readiness to commit to program development and participation in technical assistance grounded in reflective practices.  Commitment to a first meeting with the CORE consultants and RESource on one of these assigned dates in April and May (April 8, 21, May 4, 5 or 11), with commitments to follow up meetings in July or August, and September or October.  Evidence that the team will practice new approaches, strategies and conversations between meetings.  Evidence of a commitment to use new approaches, strategies and conversations. throughout the early intervention process with at least 3 families.  Goals and activities that demonstrate program capacity to implement change.  Commitment to outreach and sharing.  Evidence that funds will be used to support team involvement to meet mini-grant activities.  Evidence of ideas for sustainability of activities and practices after funding concludes.

2. Interest (15): In 2 to 3 paragraphs describe why your team is interested in participating in this mini-grant application. Describe your team’s current capacity and readiness to commit to engaging in the mini-grant process. Consider your strengths and what you are most proud of.

3. Philosophy and Practices (15): Briefly describe your team’s current philosophy and practices regarding the provision of resources, services and supports to children and families serviced by your program. Include information about how IFSP development and the other early intervention processes have contributed to your team’s approach to these practices.

4. Goals and Outcome (20): Describe 2 to 3 goals and type of changes your team expects to accomplish (outcomes) as a result of participating in this mini-grant. Identify strategies to review current practices, identify changes and implement new practices to achieve your goals. Include projected timelines and ideas about how you know you will be successful. Consider:  What supports and resources will help you achieve your goal?  What you want to accomplish toward implementing increased relationship-based practices into your team’s early intervention processes? child find and referral, intake, first contacts, service coordination, evaluation and eligibility, IFSP development, ongoing assessment, provision of services, resources & supports, and transitions.  Your plan for implementing natural support strategies to increase children and families connection to their Community, Opportunities for Relationships and Reciprocity. 

5. Challenges (10): In 2 to 3 paragraphs, describe challenges currently facing your program? How might these challenges influence or impact your work toward accomplishing the priorities of this mini-grant application?

6. Dissemination (5): For statewide dissemination, you will be asked to share information during at least one Wisline and one regional meeting. Describe at least 3 other

ways that your team will share or be available to share information about your experiences and progress with the implementation of relationship-based interventions, including natural supports, into your early intervention process.

7. Budget (10): Briefly describe how your team will utilize funds from the mini-grant to ensure successful implementation of your proposed activities. You may attach a spread sheet. Grant money can be used to cover:  personnel time for participation in mini-grant activities  CORE facilitator meetings and team planning time  phone meetings with Birth to 3 RESource facilitator  materials to assist with mini-grant activities  postage  snacks for a meeting or community training, and mileage  visits to other programs to learn about their practices
