1. Describe the behavior management system in the classroom, including positive reinforcers and consequences. Is it appropriate for the student or does it need to be modified? Rules for the whole school are listed on a poster on the wall. Students are very structured and they know all systems for accessing materials, etc.
2. Are specific positive behavior supports utilized for the student? Yes No Describe: Nonverbal cueing, verbal cueing, and reinforcing language. John Paul also has a BSP that outlines these supports.
3. Is there appropriate safety equipment in place? Yes No
4. Are appropriate safety and medical procedures being used? Yes No
5. Does it appear appropriate training has been provided? Yes No

A. Student’s behavior in independent activities
1. Describe the student’s interaction with peers. John Paul did not interact with his peers in this lesson except for the peer editor. He interacted appropriately while on-task.
2. Describe the student’s interaction with nonclassroom staff in a less structured environment? Did not observe this in this setting.
3. What activities does the student choose during breaks? No breaks during this observation.
4. What problems are evident? At times, John Paul stares into space when he is expected to work independently unless cued.
