CCS Trainees: Annual Progress Report and Renewal Application Due: Monday, February 4, 2013 Please submit electronically to [email protected]

Every year, NIH requires a progress report on the training grant. The report includes information on budgetary and other administrative matters. Most significantly, it requires information on the progress of trainees. It is important that all trainees carefully fill out and return this questionnaire on time. Please notice the requirement that your advisor review and approve your summary of research activities.

If you are eligible for funding during 2013-2014 and wish to remain in the training program:  Students in their first year of funding must also complete the last page of this form entitled Renewal Application. The selection committee will review your progress report and this additional information.  Students in their second year of funding are eligible for a competitive renewal. Therefore, they must complete a new application (found at http://www.cogsci.umn.edu/train/applying.htm). The selection committee will review your progress report and your application.

Students in their first year of funding through the cog sci traineeship who wish to remain in the training program must include the following additional items:

1) Unofficial transcript of grades through Fall Semester 2012.

2) One-page Research Plan, indicating research and academic goals for the next year of funding.

In addition, students in their first year of funding through the cog sci traineeship who wish to remain in the training program should solicit recommendations from program advisor and co-advisor (to be sent separately). These should be substantive letters of support that speak to the applicant’s qualifications and potential, as well as progress to date in the traineeship. The letters should clearly demonstrate how both the advisor and co-advisor are and will continue to be contributing mentors. The advisor and co-advisor need to give assurances that appropriate research facilities will be made available to the student. These letters should be emailed directly to the CCS office ([email protected]).
