University of Southern Nevada College of Nursing Nursing Skills Lab Guidelines For Blocks 2.0, 3.0, 4.0 and 6.0

Welcome to the Nursing Skills Lab. The Skills Lab is where you will be introduced to new skills, be expected to practice and be evaluated on your skills.

What to wear to practice in the lab

 Street clothes with lab coat

 Appropriate comfortable shoes

 Name Badge

 No excessive jewelry No long earrings, large rings on fingers or bangles Only one pair of stud earrings  Long hair must be pulled back or put up above the collar

What to wear for skill assessment check-offs (assessment/video tapping)

 Uniform with lab coat

 Appropriate shoes - nurses shoes or white only athletic shoes

 Name badge

Additional items to always bring to the skills lab for both practice and skills check-off.

 Blood pressure cuff

 Stethoscope

 Watch with second hand

 Penlight

 Black ink pen Expected behavior in the Nursing Skills Lab during videotaping

 Come in to the lab in an organized fashion and wait for instructions.

 Come prepared. Use time management skills. If you have 30 minutes to tape a skill, this does not mean 35 or 40 minutes. Be considerate of fellow classmates who also need to tape.

 Be courteous of others. Keep voices low and quiet.

 Always clean up after yourself. As a group member, help your group clean up, leaving the lab area cleaner than you found it.

Required assignments to complete before all skills lab demonstrations, practice or skills check-offs

 Complete the ATI skills modules as applicable Complete the pre-test Watch the ATI video Complete the post-test  The post-test must be passed with a 90% or greater score. The certificate of completion of the post-test must be turned in to the lab coordinator on the day of the scheduled videotaping. The certificate of the post-test is the required ticket to videotape your skill assessment.

 Some lab assignments will utilize the Lippincott’s Video Series

Lab Time

 Skills lab will be held every afternoon from 12:00 to 3:00 pm in less otherwise stated in the syllabus or block calendar. This time is for learning and demonstrating the psychomotor skills required for clinical experience.

 Demonstration and practice during this time will be done in small groups.

Additional open lab time to practice skills

 During blocks that have skills lab, the nursing skills lab will be open from 1500 to 1700, Monday through Friday. The open lab time is available for students to practice skills individually or in small groups.

 IT IS HIGHLY RECOMMENDED to take advantage of this time to practice and prepare for videotaping assessments and clinical experience.  There is a sign up notebook with a limited number of spaces for this practice time. Plan on signing up early.

 Nursing skills lab coordinators, Professor Menasche and Professor Clarren are in the lab during this time to answer questions or assist as needed.

Videotaping Skills Assessments

 Practice the skill many times before the actual scheduled videotaping. Videotaping times are not for practice. If you are not prepared and/or are taking too long, you will not be allowed to continue and will need to reschedule.

 The post-test must be passed with a 90% or greater score. The certificate of completion of the post-test must be turned in to the lab coordinator on the day of the scheduled videotaping. The certificate of the post-test is the required ticket to videotape your skill assessment.

 At completion of the videotaping, you will receive your DVD. You will view your DVD and complete the skills check-off assessment. If you are satisfied with the DVD, return the DVD with your assessment sheet to the lab coordinator. If you are not satisfied with the DVD, you may schedule a time to redo the videotaping of the skill. If you do not pass the skill at 90% or greater, you will need to remediate the skill with a lab coordinator and schedule a remediation videotaping.

 Remember to review appropriate dress and appearance. No one will be permitted to videotape who is not in uniform with appropriate shoes and professional appearance.

 Remember to bring all personal lab supplies needed for videotaping like stethoscope, watch with second hand, protective eye wear etc.

 Videotaping is usually done in pairs with one student performing the skill, the other student videotaping. Then exchange roles. Block 2.0, Health Assessment, is the exception. Three students are needed, one for the patient, one for the nurse (the person being assessed) and one videotaping.

Scheduling skills assessment videotaping

 You will need to arrange with your classmates what date and time you want to schedule your videotaping.

 A master sign up list with dates and times for videotaping will be posted in the classroom the week before the taping. The names of both (or 3) students must be included.

Videotaping for Block 2.0 Health Assessment  One skills assessment at the end of the block.

Videotaping for Block 3.0 Fundamentals

 One skills assessment of sterile technique (dressing change or foley catheter insertion).

Videotaping for Block 4.0 Adult Health I

 Two skills assessments (medication administration and NG tube insertion).

Videotaping for Block 6.0 Adult Health II

 Two skills assessments (starting an IV infusion with an IV piggyback, discontinuing an IV and tracheostomy care with suctioning).

Each videotaping will start with the student verbalizing or demonstrating the following:

 The procedure that will be performed and the reason for the procedure.

 The health care provider’s order was reviewed and the facility’s procedures and policies were reviewed.

 The patient record was reviewed for patient allergies.

 All supplies have been checked for expiration dates and the packaging is intact.

 Hand washing (or use of hand sanitizer) in front of the patient before starting the procedure.

 Full name (first and last name), student nurse at University of Southern Nevada.

 Identification of patient using two identifiers (ask the patient’s name, verify identification band).

 Explanation of the procedure to the patient including why the health care provider ordered it, what the patient might feel during the procedure.

 Any precautions, safety issues or side effects.

 Confirmation with the patient regarding allergies. Ask the patient about any allergies to iodine, shell fish or tape.  Assessing the patient for pain/discomfort.

 Positioning patient in a comfortable working height and position for you.

At the end of each videotaping student will:

 Explain any side effects or safety precautions to the patient.

 Position patient and bed back to a safe position for the patient.

 Make sure patient can reach the call button.

 Inform the patient when you will return.

After videotaping the ‘nurse’ will document a nursing note regarding the procedure. The classmate, who was videotaping, will clean up the area and gather the supplies for his/her skills assessment. The nursing note should include:  Date and time  Assessment of the patient before the procedure  The procedure  Assessment of the patient after the procedure (how the patient tolerated the procedure)

At the end of each chapter in the textbook is an area on documentation that gives examples of what is important to include in documentation.
