2010 WYRED & CONNECT Summer Day Camp

Last updated 040615 2016 Summer Camps Day Camp: June 20 – August 19, 2016 Nine (9) Week Program Camp 8:30 am to 4:30 pm

Students Entering 2st thru 8th Grades (Child must be 7 years old at the start of camp.) - Children and families will find a safe, fun, productive environment where they continue to grow in mind, body and soul during the summer. We will provide programs, activities, opportunities, resources and loving hands doing:

*Christian Education * Nutrition Education *Computer Lab *Arts & Crafts *Character Building *Community Service *Swimming *Science *Sports * Games *Enrichment Music *Creative Engaging Field Trips *Team Building * Reading & Math Development * Drama

LOCATION: The Salvation Army - 445 Madison Street, Waukesha, WI 53188

FEES: Registration Fee: $ 10.00 per child (If the child attends 3 or more weeks of camp this will be applied to field trip costs) Camp Fee: $ 75.00 per week per child ($675.00 nine weeks) Family Rate: $ 60.00 ($540.00 nine weeks) each additional child Before and After Care: $ 10.00 per child, per week, per extension $ 3.00 per child, per day, per extension (7:30 am- 8:30 am and/or 4:30 pm - 5:30 pm) Fees include Lunch, Snacks and most Field Trips

PAYMENT OPTIONS: A. Pay tuition for the full summer in by June 15th and receive a 10 % discount. B. Payment Plan: Agreed upon payment must be made in full at least one week in advance or by the Friday prior to attending. Slots will be held only when fees are paid.

FORMS NEEDED:  Child Registration with Health History and Emergency Contacts (Form 2 - One per child)  Registration Agreement including Code of Conduct/Consent to Publish (Forms 3 and 4- One per family)  Application for Financial Assistance (Form 5 - If applicable one per family)

SUBMIT completed forms to The Salvation Army Waukesha Youth Department with $10 registration fee per camper.

ADDITIONAL FORMS and FEES:  City of Waukesha Parks, Recreation, Forestry – Annual Financial Assistance Application for Recreation Programs Our program will include swimming at Horeb this summer with a pool fee (add’l to camp fee) for each visit. Depending on your circumstances it may be advantageous for you to purchase a child or family pool pass. Pool passes can be purchased through the Waukesha Parks and Recreation Department. (Two WP&R forms are necessary for purchase.)

 Field Trips –Once field trips have been determined we will create a permission slip for parent signatures. Most field trips will be included in the camp fee. However some additional fees may occur for select trips.

(If your child is attending SA Day Camp and would like to attend the appropriate residential camp the cost is the same for that week and there are no additional registration fees.)

RESIDENTAL CAMP FEES: Registration Fee: $ 10.00 per child, non-refundable/non-transferable. Camp Fee: $ 75.00 per camp Transportation for residential camps provided from The Salvation Army Waukesha. Deadline to register for residential camps: May 30, 2015. After deadline check for availability, if that camp is full your child can be placed on a waiting list.

Slots for all camps will be held when required paperwork is complete and registration fee is paid.
