NEW BROCKTON HIGH SCHOOL ADMISSIONS PROCEDURES

Welcome to Coffee County Schools! Students wishing to enroll in New Brockton High School should follow the following procedures. Please complete the following checklist. All accompanying documentation must be presented at the time of enrollment. If you have questions please call the Main Office at 334-894-2350. Thanks!

_____ 1. Withdrawal documentation from previous school, including Unofficial transcripts confirming grade placement, transfer grades, discipline report, and attendance report

_____ 2. Alabama Certificate of Immunization (Blue Card)

_____ 3. Social Security Card (if possible)

_____ 4. Proof of Residence(2) MUST include One of the following:  Property tax records _____  Current Power bill______ Mortgage documents or property deeds _____  Residential lease _____  Driver’s license _____  Voter precinct identification _____  Automobile registration or insurance _____

_____ 5. Documentary Proof of Custody (Must Provide One)  Student’s birth certificate _____  Court degree declaring that the school zone resident is the student’s legal “primary” guardian, adoptive parent, or court-appointed custodian; or appropriate foster parent documentation. A stamped copy of such decree must be presented to school officials prior to enrollment. _____

_____ 13. Lunchroom Application- Completed and Returned to school

_____ 14. Agricultural Employment Survey-Completed and Signed

_____ 15. Textbook and Internet usage form-Completed and Signed

_____ 16. HOPE Survey- Completed and Returned to school

DISCLAIMER: A student may not be denied enrollment due to lack of Social Security Card or Birth Certificate if they are migrant, ELL, neglected, homeless or delinquent
