Foreign & Gourmet Foods Parent Permission Form

Dear Parent or Guardian:

This semester, your son/daughter has signed up to take Foreign & Gourmet Foods. As part of this curriculum, your son/daughter must pass a safety/sanitation test with a 90% before he/she will be allowed to participate in foods labs.

As part of the class, your son/daughter will be participating in foods labs, and will be working with kitchen utensils and appliances. Any deliberate misuse of kitchen utensils or appliances will result in the loss of lab privileges and a referral to the office.

During foods labs, all jewelry including facial & tongue piercings must be removed. Body glitter must not be worn. Doing do will result in the loss of lab privileges for that day, and an alternate activity will be assigned.

If, at any time your son/daughter participates in horseplay of any kind, including but not limited to, pushing, running, threatening, practical jokes, eating food not assigned to him/her, deliberately ruining food, towel snapping, or deliberate misuse of kitchen utensils (i.e. using them as weapons), he/she will immediately lose lab privileges and be referred to his or her AP.

Your son/daughter must maintain a passing grade to participate in foods labs. If he/she does not have a passing grade on lab day, he/she will spend the lab time making up missing work.
