Notes: (1) Please note that ethical approval must be obtained prior to any data collection or analysis taking place. (2) Ethical approval is time-limited, normally to be granted initially for one year for MSocSc in Psychology dissertation, and two years for MSocSc in CP/EP dissertation and PsyD CP/EP Thesis. Justifications for extension must be provided in the “Amendment of an Approved Ethics Form”. (3) The completed application form, together with all related documents, should be sent to the General Office, Department of Psychology (Rm627, Jockey Club Tower, Centennial Campus). The submission of incomplete application materials will delay the ethics review process. (4) No HREC reference number will be assigned to your research ethics application. Department will send you an e-mail informing you the application result which serves as a proof of your research ethics application being approved by the Departmental Research Ethics Committee. Please be reminded to delete the line about “HREC Reference Number/HREC Approval Expiration Date” in the Informed Consent Form/Debriefing Form.
