I am writing to you as a Team Leader Midwife of the Antenatal Clinic of Worthing Hospital. New NHS guidelines regarding pregnancy screening are coming into effect in next few months. The main changes are rolling out of Family Origin Questionnaire (FOQ) and availability of combined screening for Down's. What it all means is that women need to access the maternity service early in pregnancy to allow booking at 8-10 weeks. In preparation to these changes we would like to receive all our referrals as early as possible and via e-mail to avoid any delay.

Any kind of referral should have the following information: First name and surname Date of birth Address and the postcode Contact tel no GP name and practice Date of lmp (last menstrual period)
