2016 JUNIOR FAIR ENTRY FORM **PLEASE LIST ONE MEMBER PER ENTRY LINE** Please read back of form for instructions This form must be completed by the 4-H/FFA Advisor & taken to the SR. Fair Board Office (on fairgrounds) between August 29-Sept 2 & Sept 6-Sept 9 between 9:00am and 4:00.m. This form is for entry of the following projects: livestock, horses, dogs, companion animals, crops, cakes, pies, candy, and grazing. Look on back for instructions on how to fill out this entry form. Please complete all information and double check for accuracy. See sample entry to help you get started.

If you have a member that is not a member in good standing (that you are intentionally NOT entering a project for) please indicate this on the entry form and call the Extension Office. IMPORTANT: Please complete the entry form in its entirety using one line per entry. Fair Bd Use only Exh # Senior Fair Board assigns this # for each exhibitor Exhibitor’s Last Name Please print clearly members last name Exhibitor’s First Name Please print clearly members first name Parent’s Name Please print clearly member’s parents full first and last name (especially if last name is not the same) Address, City, zip Please print full address including City and zip code Fair Age This is the member’s age at fair time School Name of school your member attends (not the school district we send letters to principals for fair excuses) (include address of school on a separate paper if school is not in Fairfield County) Book # Use the Junior Fair Book to determine what book number (ex: Book 7 -4-H/FFA Pygmy Goat) Class # Use the Junior Fair Book to determine what class # the member’s project belongs in ex: Pygmy Goat Classes: 1. Wethers under 6 mos. What Breed of (cattle, Sheep etc.) Beef-ex: Angus, Sheep-ex: Dorset, Rabbit-ex: Rex, Poultry-ex: Barred Rock Scrapie Tag # (for sheep & goats) This is usually found tattooed in the ear or tail of goats and is a tag in the ear for sheep.

Entries submitted after the deadline put your members at risk of being disqualified. Please be sure your club is not excluded by a late entry submission. Thank you. You may make additional copies of this form as needed or contact OSU Extension - Fairfield County for additional copies.

Members will receive premiums on only their two (2) highest placing entries. Please note that all projects which have not been judged must be entered. These include: Cake Decorating, Pies, Candies, Dogs, Small Animals, Other Creatures, Grazing, Crops, Gardening and all Livestock projects. Projects that were graded in the summer do not need to be entered. (self-determined cake decorating judged during the summer does not make an entry refer to members handbook) DIRECTIONS TO FAIRGROUNDS OFFICE: Enter fairgrounds through the Broad Street gate and make an immediate right. Proceed to the red brick office building. Or mail entries to Attn: Junior Fair Entries P O Box 945 Lancaster Ohio 43130 must be received by September 9th.
