REQUEST FOR CENTRALIZED PASSPORT ACTION Michigan Department of Health and Human Services
Instructions: An authorized representative of your Friend of the Court (FOC) office must complete the information below if your office is requesting that Office of Child Support (OCS) Central Operations handle your passport denial actions centrally as described in Section 6.24, “Passport Denial/Restriction” of the Michigan IV-D Child Support Manual.
Per Section 6.24, OCS Central Operations and local FOC offices will follow the same statewide guidelines for withdrawing non-custodial parents (NCPs) from passport denial. If an NCP has multiple dockets and one of the dockets is in a county that has chosen to centralize passport denial actions, OCS Central Operations will coordinate with all counties when the NCP negotiates to pay less than 25% of his/her unpaid ordered amount in an employment-related situation or the case qualifies to deviate from the guidelines.
Updates to your FOC office’s passport contact information should be made on mi-support’s Partner Contact Information page.
FOC: Please complete the following:
Name of county Street/P O Box
City Zip code Phone
Signature of FOC or Authorized Representative Date
Printed name Title
Phone Fax Email
Please return this completed and signed form via fax to OCS Central Operations at 517-335-3030, or scan it and email as an attachment to [email protected].
