VIRGINIA BOARD OF NURSING INFORMAL CONFERENCES May 7, 2014

TIME AND PLACE: The informal conferences held before an Agency Subordinate of the Board of Nursing were convened at 9:20 A.M., on May 7, 2014 in Hearing Room 5, Department of Health Professions, 9960 Mayland Drive, Suite 201, Richmond, Virginia.

AGENCY SUBORDINATE: Jane Elliott, RN, PhD

STAFF PRESENT: Ann Tiller

OTHERS PRESENT: Peter Opper, Adjudication Specialist

CONFERENCES SCHEDULED: Titilayo Akadiri, CNA 1401-155846 Ms. Akadiri appeared, accompanied by Bea Ogunware, her sister.

Ms. Akadiri was informed a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Akadiri unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Akadiri within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated. CONFERENCES SCHEDULED: Adrian Edwards, CNA 1401-1574444 Ms. Edwards appeared.

Ms. Edwards was informed a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Edwards unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Edwards within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.

Based upon information provided by Mr. Opper, Dr. Elliott ruled that adequate notice was provided to Ms. Graham.

A recommended decision will be made and mailed to Ms. Graham within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order. CONFERENCES SCHEDULED: Catheia Wilcox, CNA 1401-150879 Ms. Wilcox appeared, accompanied by Henry McNair, her father.

Ms. Wilcox was informed a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Wilcox unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Wilcox within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated. CONFERENCES SCHEDULED: Debra Amaker, CNA 1401-097943 Ms. Amaker appeared, accompanied by Shirley T. Hodges, her friend and co- worker.

Ms. Amaker was informed a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Amaker unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Amaker within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated. CONFERENCES SCHEDULED: Erin Hoemman, CNA 1401-158074 Ms. Hoemman did not appear.

Based upon information provided by Mr. Opper, Dr. Elliott ruled that adequate notice was provided to Ms. Hoemman.

A recommended decision will be made and mailed to Ms. Hoemman within ninety days.

2 This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order. CONFERENCES SCHEDULED: Brandi Salvato, CNA 1401-082499 Ms. Salvato did not appear.

Based upon information provided by Mr. Opper, Dr. Elliott ruled that adequate notice was provided to Ms. Salvato.

A recommended decision will be made and mailed to Ms. Salvato within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order. .CONFERENCES SCHEDULED: Sheila M. Tinsley, CNA 1401-006951 Ms. Tinsley did not appear.

Based upon information provided by Mr. Opper, Dr. Elliott ruled that adequate notice was provided to Ms. Tinsley.

A recommended decision will be made and mailed to Ms. Tinsley within ninety days.

This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall be a Final Order. CONFERENCES SCHEDULED: Rhonda Austin, CNA 1401-129018 Ms. Austin appeared accompanied by Deborah Williams, her mother-in-law.

Ms. Austin was informed a recommended decision will be made and mailed to her within ninety days. This recommendation will be presented to the full Board; if accepted, an Order will be entered. As provided by law, this decision shall become a Final Order thirty days after service of such order on Ms. Austin unless a written request to the Board for a formal hearing on the allegations made against her is received from Ms. Austin within such time. If service of the order is made by mail, three additional days shall be added to that period. Upon such timely request for a formal hearing, the Order shall be vacated.
