Summary of Service

The Rapid Access to Carers at the end of life (RACE) service aims to provide a rapid response Hospice at Home night service for Worcestershire to prevent hospital admission or to expedite same day hospital discharge for patients at the end of life when all other appropriate sources of support are unavailable. Carers will be on call as per a planned rota, and able to respond to a request for care at any time of the night, although it is anticipated that most shifts will have been pre-arranged during the day. The service will provide care until an alternative care package is arranged, up to a maximum of 48 hours. Carers will be employed by each of the three Worcestershire Hospices to complement their individual Hospice at Home services.
