Job Description
Title:- Administrator, RACE team/Primrose at Home
Hours:- 22½ per week
Nature of Post:- Fixed term contract, 6 months
Reports to:- Primrose at Home Team Leader
Responsible to:- Nurse Manager
Summary of Service
The Rapid Access to Carers at the end of life (RACE) service aims to provide a rapid response Hospice at Home night service for Worcestershire to prevent hospital admission or to expedite same day hospital discharge for patients at the end of life when all other appropriate sources of support are unavailable. Carers will be on call as per a planned rota, and able to respond to a request for care at any time of the night, although it is anticipated that most shifts will have been pre-arranged during the day. Requests for shifts at home will be generated by a District Nurse who will have completed a risk assessment of the patient and home environment. The service will provide care until an alternative care package is arranged, up to a maximum of 48 hours. Carers will be employed by each of the three Worcestershire Hospices to complement their individual Hospice at Home services.
The service is to be coordinated for the county during working hours by means of a single point of access based at Primrose Hospice. Out of hours it will be coordinated by the Out of Hours Service with all referrals to be risk-assessed by the night District Nurses.
Summary of Role:
As a member of the administrative team at Primrose Hospice the postholder will assist the Primrose at Home Team Leader in coordinating a 6 month pilot of the RACE service as set out above The postholder will be required to coordinate requests for shifts for patients across the county, and will also be required to set up systems for capturing activity data and to support evaluation of the success or otherwise of the project
1. Main duties
To act as a single source of access for urgent end of life care for Worcestershire patients during working hours
To receive and coordinate requests for care, ensuring that referral criteria has been met and all other sources of support, including the Hospice at Home Services based at KEMP, Primrose and St Richard’s Hospice are unavailable
RACE Administrator July 2010 1 To develop appropriate links and networks with other home care providers so that care can be arranged with the minimum of worry for the patient and/or family
To verify that a risk assessment and care plan has been completed and that sufficient information is available to enable the Carer to attend and provide safe care to the patient and/or family
To liaise with patients,carers and Health Care Professionals, demonstrating excellent communication skills and appropriate sensitivity to the patient/family group supported by the Hospice
To maintain and update the rota of RACE availability for out of hours services
To set up systems for capturing activity data and for the evaluation of the project
To input data onto the palliative care electronic notes system (SystmOne) as required, including registering patients and entering written records of telephone and other contacts
To coordinate induction and training sessions for RACE carers from all three Hospices
2. General
To take minutes for all meetings within and outside of the Hospice relating to RACE service and Primrose at Home, maintaining strict confidentiality at all times
To produce leaflets/fliers/newsletters as required for the service
Organising events and meetings relating to the service
3 Personal Education and Development
Be aware of up to date information relevant to own role within the Hospice. Keep up to date with new technology
Attend appropriate courses to maintain own skills and expertise
Attend in-house training sessions on aspects of Health and Safety and other issues relevant to role
