How to Get Your Providence Health Care ID

School identification must be worn and visible at all times while on placement at PHC. Only instructors and preceptorship students requiring access to locked units will be granted PHC identification unless otherwise indicated.

If you require ID, please fill out the Access Request Form. A manager or operations leader will need to approve your request. They can do this by Photo ID Access sending an email to [email protected]. Request Form

Even though you will receive a PHC ID, you are still requested to wear your school ID as well.

Option #1  Employee brings Government issued picture ID to the St. Paul’s Hospital Security Office, Room B19, in the Burrard Building basement, on Tuesday between 08:00-11:00 or Thursday between 11:00-14:00  Appointments are optional during these times, but individuals with appointments will be served first  The Photo ID badge is produced, programmed and put into inter-office mail within 5 business to be picked up: o St. Paul’s Hospital will be delivered to the Security Booth in the Emergency Department. ID’s are available for pick-up 24/7 (subject to urgent tasks requiring Security to leave the booth) o Mount St. Joseph’s will be sent to the Security Office o Langara, Brock Farhni, Holy Family, Honoria Conway and Youville will have the ID sent to Reception o Other facilities not listed, the ID must be picked up from St. Paul’s Security Booth  Appointments may be made through the Security Supervisor, 604-806-9439, or via email, [email protected]

Option #2  Employee brings Government issued picture ID to the PHC Photo ID Office at Vancouver General Hospital. The office is in Room 179 of the Willow Chest Centre, 2647 Willow St., Vancouver and is open Monday - Friday 08:00-15:45, closed 12:00-13:00 for lunch  Appointments are mandatory  Picture is taken, ID badge produced, programmed and provided to the employee immediately  Appointment requests/questions should be directed to 604-875-4111 x.20003, or via email, [email protected]
